专科医师综合干预对重性精神障碍患者康复效果影响的对照研究

摘要
目的探讨精神专科医师综合干预对重性精神障碍患者康复效果的影响。方法将符合入组标准的97例重性精神障碍患者随机分为干预组和对照组,干预组为47例,对照组为50例,对干预组患者应用精神专科医师干预模式进行干预,对照组患者接受社区常规电话随访服务,以便及时发现患者病情不稳定等情况。采用阳性和阴性症状评定量表、自知力与治疗态度问卷、个人和社会功能量表、焦虑自评量表、抑郁自评量表以及康复状态量表在入组前以及入组后每三个月评估一次。结果以基线和干预六个月后为观察点,基线水平两组各项评分差异无统计学意义(P>0.05);干预后干预组的个人和社会功能评分为(68.9±15.6)分,显著高于对照组的(61.8±14.3)分,差异显著(t=2.20,P<0.01);症状量表总分干预组评分为(47.0±18.0)分,明显低于对照组的(56.6±22.0)分,差异显著(t=-2.319,P<0.05)。结论精神专科医师综合干预模式能提升患者社会功能,改善患者的精神症状,对患者的康复有利,延长观测时间可能会发现精神专科医师综合干预模式更多的效果。
Objective To explore the effect of comprehensive intervention of psychiatrists on rehabilitation of patients with severe mental disorders. Methods Ninety-seven patients with severe mental disorders were randomly divided into control group(50 cases)and intervention group(47 cases). The patients in the intervention group were comprehensively intervened by psychiatrists. Patients in the control group received the regular telephone followup service of the community,so as to find out the patient's condition in time. Using Positive and Negative Syndrome Scale,Insight and Treatment Attitudes Questionnaires,Personal and Social Performance Scale,Self-Rating Anxiety Scale,Self-Rating Depression Scale and Morningside Rehabilitation Stats Scaler to evaluate every three months. Results Setting baseline and intervention after six months as the observation point,the results showed that there was no significant difference between the two groups in the baseline(P>0.05). After intervention,the scores of PSP in intervention group were(68.9±15.6),which were significantly higher than that of the control group(61.8±14.3)(t=-2.319,P<0.05). The score of symptom scale was significantly lower in the study group(47.0±18.0)than that in the control group(56.6±22.0)(t=-2.319,P<0.05). Conclusion Comprehensive intervention of psychiatrists can enhance the patient's social function,improve the patient's mental symptoms. This model is benefit for the rehabilitation of patients with severe mental disorders in community. Prolonging the observation time may find more effects.
