摘要
目的:探讨自锁托槽与传统金属托槽后牙段扩弓的应用效果。方法:选取2015年6月至2017年3月苏州大学附属张家港医院收治的82例正畸治疗患者为研究对象,随机分为研究组(52例)和对照组(30例)。对照组采用传统金属托槽治疗,研究组采用自锁托槽治疗。比较两组治疗前、后锥形束CT(CBCT)测量的牙弓、牙槽骨、基骨等指标,并评估患者治疗前、后的牙周健康情况。结果:扩弓后,两组第一磨牙牙弓宽度,第一、第二前磨牙牙弓宽度、尖牙宽度以及基骨宽度均显著增大,第一磨牙腭侧牙槽骨厚度明显增厚,第一磨牙颊侧牙槽骨厚度显著变薄,且第一磨牙倾斜度明显增大,牙周健康指数明显上升,与扩弓前比较差异均有统计学意义(均P<0.05);观察组第一、第二前磨牙宽度增大幅度小于对照组(P<0.05),两组牙弓、基骨宽度、牙槽骨改变及牙周健康指数等指标比较,差异均无统计学意义(均P>0.05)。结论:传统托槽与自锁托槽对牙弓、牙槽骨及基骨等牙弓形态及牙周健康的影响基本相当,正畸效果无明显差异。
        Objective:To explore the effect of self-locking brackets and MBT straight wire application in posterior teeth patching.Methods:From June 2015 to March 2017,a total of 82 patients who received orthodontic treatment in our hospital were selected,and randomly assigned to use either MBT straight wire(control group,n=30)or self-ligating system(observation group,n=52)during posterior teeth patching.The width of dental arch and dental base bone and alveolar bone thickness were detected by CBCT before and after treatment.The periodontal health of patients was evaluated.Results:After arch expansion,the arch width of the first molar,the arch width of the first and second premolar teeth,canine width,the width of dental base bone,the periodontal health index,first molar inclination and first molar buccal alveolar bone thickness were significantly increased,while the first molar palatal lateral alveolar bone thickness was decreased in both groups(P<0.05).The increases in the first and second premolar width were more significant in the observation group(P<0.05).There was no significant difference in dental arch,basement width,alveolar bone changes and periodontal health index between the two groups(P>0.05).Conclusion:MBT straight wire and self-locking brackets had similar effects on the shape of dental arch,dental base bone and alveolar bone and periodontal health,and similar orthodontic effect.
