新型鼻塞持续气道正压呼吸治疗新生儿呼吸窘迫综合征临床分析

摘要
目的对新生儿呼吸窘迫综合征患儿应用新型鼻塞持续气道正压呼吸治疗的临床疗效进行探究。方法该次研究纳入97例自2016年10月—2017年9月间该院收治的新生儿呼吸窘迫综合征患儿,依照患儿应用治疗方式的差异将其分为常规组和新型组,给予所选患儿基础治疗,常规组患儿在此基础上应用常规机械通气治疗,新型组患儿在此治疗基础上应用新型鼻塞持续气道正压呼吸治疗,对比患儿动脉血p H值、OI、Pa CO2、机械通气治疗时间、氧疗时间、开奶时间和住院时间。结果新型组Pa CO2为(42.30±2.25)mm Hg,常规组为(46.62±2.90)mm Hg,差异有统计学意义(t=8.185 4,P=0.013 6),新型组p H值为(7.43±0.08),常规组为(7.24±0.11),2组差异有统计学意义(t=9.712 1,P=0.023 8),新型组OI值为(262.54±105.13)mm Hg,常规组为(210.46±101.41)mm Hg,2组差异有统计学意义(t=2.4834,P=0.014 8)。新型组机械通气时间为(2.62±2.11)d,常规组为(6.57±2.94)d,2组差异有统计学意义(t=7.588 4,P=0.023 7),新型组氧疗时间为(16.45±11.56)d,常规组为(27.52±13.79)d,2组差异有统计学意义(t=4.280 2,P=0.037 6),新型组开奶时间为(3.57±0.32)d,常规组为(6.91±0.40)d,2组差异有统计学意义(t=45.353 5,P=0.029 8),新型组住院时间为(12.38±2.52)d,常规组为(18.47±4.26)d,2组差异有统计学意义(t=8.546 8,P=0.024 1)。结论对新生儿呼吸窘迫综合征患儿应用新型鼻塞持续气道正压呼吸治疗能够使患儿血气分析指标得到改善,同时还能够使机械通气治疗时间等得到缩短,有助于加快患儿病情好转。
        Objective To study the clinical curative effect of nCPAP in treatment of respiratory-distress syndrome of newborns. Methods 97 cases of newborns with respiratory-distress syndrome admitted and treated in our hospital from October 2016 to September 2017 were selected and divided into two groups, according to different treatment method,all children used the basic therapy, and the routine group used the routine mechanical ventilation, while the newly group use the nCPAP on the basis of the control group, and the arterial blood p H value, OI,PaCO_2, mechanical ventilation treatment time, oxygen therapy time, milk open time and length of stay were compared between the two groups.Results The PaCO_2 in the newly group and in the routine group was respectively(42.30±2.25)mm Hg and(46.62±2.90)mm Hg(t=8.185 4,P=0.013 6), and the PH value in the newly group and in the routine group was respectively(7.43±0.08) and(7.24±0.11), and the difference was statistically significant(t=9.712 1,P=0.023 8), and the OI in the newly group and in the routine group was respectively(262.54±105.13)mm Hg and(210.46±101.41)mm Hg, and the difference was statistically significant(t=2.483 4,P=0.014 8), and the mechanical ventilation time in the newly group and in the routine group was respectively(2.62±2.11)d and(6.57±2.94)d, and the difference was statistically significant(t=7.588 4,P=0.023 7), and the oxygen therapy time in the newly group and in the routine group was respectively(16.45±11.56)d and(27.52±13.79)d, and the difference was statistically significant(t=4.280 2,P=0.037 6), and the milk open time in the newly group and in the routine group was respectively(3.57±0.32)d and(6.91±0.40)d, and the difference between the two groups was statistically significant(t=8.546 8,P=0.024 1). Conclusion The application of n CPAP in treatment of respiratory-distress syndrome of newborns can improve the blood gas analysis and shorten the mechanical ventilation time, which contributes to accelerating the improvement of diseases of children.
