摘要
目的探讨影响全髋关节置换术(THA)患者并发髋关节脱位的相关因素。方法回顾性分析2016年1月至2019年1月我院收治的612例THA患者的临床资料。选择术后1个月内发生髋关节脱位的26例患者为观察组,未发生髋关节脱位的586例患者为对照组。比较两组患者临床资料,对影响PHA患者并发髋关节脱位的相关因素进行非条件Logistic多元回归分析。结果 Logistic多元回归分析结果显示,手术入路为后外侧、手术时间≥2 h、体位控制不当、假体在安全区外、搬运不当是影响THA患者并发髋关节脱位的独立危险因素(P<0.05)。结论 PHA患者并发髋关节脱位与手术入路、手术时间、体位控制、假体位置、搬运不当等多种因素密切相关,临床上应加以重视并采取干预措施以减少髋关节脱位的发生。
        Objective To investigate the factors affecting hip dislocation in patients undergoing total hip arthroplasty(THA). Methods The clinical data of 612 patients with THA admitted to our hospital from January 2016 to January 2019 were retrospectively analyzed. 26 patients with hip dislocation within 1 month after surgery were selected as the observation group, and 586 patients without hip dislocation were selected as the control group. The clinical data of the two groups were compared, and the unrelated conditional logistic multiple regression analysis was performed on the related factors affecting PHA patients with hip dislocation. Results Logistic multiple regression analysis showed that, the surgical approach is posterior lateral, operative time≥2 h, improper position control, prosthesis outside the safe area, improper handling were independent risk factors for hip dislocation in patients with THA(P<0.05). Conclusion PHA patients with hip dislocation are closely related to various factors such as surgical approach, operation time, position control, prosthesis position, and improper handling. Clinical attention should be paid to and interventions should be taken to reduce the occurrence of hip dislocation.
