新辅助化疗联合间隔减瘤术治疗晚期卵巢癌的临床疗效研究

摘要
目的新辅助化疗联合间隔减瘤术治疗晚期卵巢癌的临床疗效研究。方法回顾性分析2010年2月—2018年2月期间该院102例晚期卵巢癌肿瘤患者的临床资料,以治疗方式为分组依据,分为对照组和研究组,每组51例患者,对照组实施减瘤术,研究组以新辅助化疗联合间隔减瘤术治疗,观察和对比临床疗效。结果肿瘤减灭成功率的对比,研究组残留病灶<1 cm患者33例,成功率64.71%,对照组残留病灶<1cm患者25例,成功率49.02%,差异有统计学意义(χ~2=4.521,P=0.12)。患者3～5年生存率对比,研究组31例,60.78%,对照组22例,43.14%,差异有统计学意义(χ~2=5.033,P=0.06)。两组患者围术期指标对比,研究组术中出血量少于对照组,差异有统计学意义(t=5.334,P=0.06)。研究组手术时间短于对照组,差异有统计学意义(t=4.561,P=0.04)。术后并发症对比,研究组11.76%,对照组23.53%,差异有统计学意义(χ~2=4.389,P=0.02)。结论新辅助化疗联合间隔减瘤术对于晚期卵巢癌有良好的治疗效果,能够提升患者的生存率。
Objective Clinical study of neoadjuvant chemotherapy combined with interval tumor reduction for advanced ovarian cancer. Methods The clinical data of 102 patients with advanced ovarian cancer in our hospital from February 2010 to February 2018 were retrospectively analyzed. The patients were divided into control group and research group according to the treatment methods. 51 patients in each group were treated with neoadjuvant chemotherapy combined with interval neoplasia. The clinical efficacy of the study group was observed and compared. Results Comparing the success rate of tumor reduction, 33 patients with residual lesions less than 1 cm in the study group had a success rate of 64.71%, while 25 patients with residual lesions less than 1 cm in the control group had a success rate of 49.02%. The difference was statistically significant(χ~2=4.521, P=0.12). The 3-5 year survival rate of the patients in the study group was 31 cases(60.78%) and that in the control group was 22 cases, 43.14%. The difference was statistically significant(χ~2=5.033, P=0.06). The perioperative indexes of the two groups were compared. The intraoperative bleeding volume of the study group was less than that of the control group. The difference was statistically significant(t=5.334, P=0.06). The operation time of the study group was shorter than that of the control group. The difference was statistically significant(t=4.561, P=0.04). The postoperative complications of the study group were 11.76% and that of the control group was 23.53%. The difference was statistically significant(χ~2=4.389, P=0.02). Conclusion Neoadjuvant chemotherapy combined with septal subtraction has a good therapeutic effect on advanced ovarian cancer and can improve the survival rate of patients.
