PDCA循环管理法在我院中药注射剂管理中的应用效果分析

摘要
目的探讨计划、实施、检查、处理(PDCA)循环管理法在山西省汾阳医院中药注射剂管理中的应用效果。方法按照PDCA循环管理法,先收集资料、找出存在的问题,然后分析原因并拟订对策、检查和落实,最后对实施PDCA循环前后的中药注射剂销售金额及所占比、中药注射剂用药合理性以及不良反应发生率等指标进行对比。结果通过建立管理小组专门监管、修订制度并培训、制订相应奖惩措施、进行专项医嘱点评等系列措施,中药注射剂销售金额占药品总金额的比率从3. 92%下降至3. 09%,住院患者中药注射剂超适应证用药的不合理率从15. 25%下降至8. 92%,降幅为6. 33%;中药注射剂重复用药的不合理率从10. 75%下降至7. 17%,降幅为3. 58%;中药注射剂配伍不当的不合理率从7. 25%下降至2. 33%,降幅为4. 92%;中药注射剂溶媒选择不当的不合理率从6. 25%下降至4. 08%,降幅为2. 17%;中药注射剂超剂量的不合理率从4. 17%下降至3. 83%,降幅为0. 34%;超疗程用药的不合理率从3. 25%下降至3. 17%,降幅为0. 08%。结论 PDCA循环管理法用于我院中药注射剂管理成效显著,可在医院其他管理领域推广应用。
Objective To investigate the effects of plan,do,check,action(PDCA) cycle theory in the management of Chinese medicine injection in Shanxi Fenyang Hospital. Methods According to the method of PDCA cycle,firstly,materials were collected to find out the problems. Secondly,the reasons were analyzed and countermeasures were made out,checked and implemented. Finally,the indicators of TCM injection were compared before and after the implementation of PDCA,including the total cost of the drug and the money used in TCM injection,the change of irrational use of TCM injection,the adverse reaction rates of TCM injection,etc. Results TCM injection using proportion of the total cost of the drug decreased from 3. 92% to 3. 09% through establishing management group,revising systems and training,formulating relevant rewards and punishments measures,special medical advice etc. The rate of irrational drug use in the treatment of TCM injection decreased from 15. 25% to 8. 92%,a decline of 6. 33%. The rate of irrational repeated use of TCM injection decreased from 10. 75% to 7. 17%,a decline of 3. 58%,the rate of irrational compatibility of TCM injection decreased from 7. 25% to 2. 33%,a decline of 4. 92%. The unreasonable rate of solvent selection of TCM injection decreased from 6. 25% to 4. 08%,a decline of 2. 17%. The unreasonable rate of super dose of TCM injection decreased from 4. 17% to 3. 83%,a decline of 0. 34%. The irrational rate of super course of treatment of TCM injection decreased from 3. 25% to 3. 17%,a decline of 0. 08%. Conclusions The application of PDCA cycle considerably improves the management of TCM injection in our hospital. It can be widely used in other management aspects of hospital.
