二维及彩色多普勒超声对腮腺肿瘤的诊断价值

摘要
目的评价二维及彩色多普勒超声诊断腮腺肿瘤的价值。方法回顾性分析75例腮腺肿瘤患者的二维及彩色多普勒超声图像,以术后病理诊断为金标准,评价二维及彩色多普勒超声各项指标的诊断价值。结果 75例中,良性肿瘤60例,恶性肿瘤15例。二维及彩色多普勒超声诊断与病理诊断的符合率为85.33%。ROC曲线分析显示,肿瘤内部回声性质、边界、形态及内部彩色血流信号对于腮腺肿瘤良、恶性诊断有统计学差异(P<0.05),其诊断的灵敏度和特异度分别为66.67%和88.33%、46.67%和98.33%、86.67%和78.33%及66.67%和75.00%。结论二维及彩色多普勒超声对腮腺肿瘤诊断及其良、恶性鉴别具有重要价值。
Objective To evaluate the value of two-dimensional and color Doppler ultrasonography in the diagnosis of benign and malignant parotid tumors.Methods Ultrasound images of two-dimensional and color Doppler ultrasonography in 75 patients with parotid tumors were retrospectively analyzed.Taking pathological results as the golden standards,the value of ultrasonography in the diagnosis of benign and malignant tumors of the parotid gland was evaluated.Results Of 75 cases,60 cases were pathologically diagnosed as benign parotid tumors and 15 cases as malignant parotid tumors.The diagnostic accordance rate of ultrasonography with pathology was85.33%.The ROC curve analysis showed that the internal echo property,boundary,morphology and internal blood Doppler flow signal were the significant factors for differentiating the malignant from benign tumors of the parotid gland(P<0.05).The sensitivity and specificity of ultrasonography in the diagnosis of benign and malignant parotid tumors were 66.67% and 88.33% for internal echo property,46.67% and 98.33%for boundary,86.67% and 78.33%for morphology,and 66.67%and75.00%for internal blood Doppler flow signal,respectively.Conclusion Two-dimensional and color Doppler ultrasonography has an important diagnostic value in diagnosing and differentiating the malignant tumors from benign tumors of the parotid gland.
