超声引导下膈神经阻滞治疗顽固性呃逆20例临床观察

[背景]观察超声引导下膈神经阻滞治疗顽固性呃逆的疗效.[病例报告]选择自2015年10月至2017年10月间就诊的顽固性呃逆患者40例,随机分为常规组(20例)和实验组(20例),常规组给予盲探法膈神经阻滞治疗,实验组给予超声引导下膈神经阻滞治疗,比较两组阻滞成功率、有效率及并发症发生率.实验组神经阻滞操作成功率明显高于常规组(P<0.05);实验组治疗后7,30,90 d时的有效率明显高于常规组(P<0.05);治疗期间常规组出现颈部血肿3例,神经损伤2例,治疗组未出现并发症.[讨论]超声引导下膈神经阻滞治疗顽固性呃逆疗效满意.
BACKGROUND To observe the clinical effects of ultrasound-guided phrenic nerve block in the treatment of intractable hiccup. CASE REPORTS 40 patients with intractable hiccup who visited the hospital from October 2015 to October 2017 were randomly divided into conventional group(20 cases) and experimental group(20 cases). The patients in the conventional group were treated with the phrenic nerve block by landmark technique, whereas the experimental group by ultrasound guidance. The successful rate, effective rate and complication rate were compared between the two groups. The success rate of nerve block operation in the experimental group was significantly higher than that in the conventional group(P<0.05). The effective rate of the experimental group was significantly higher than that of the conventional group at 7, 30 and 90 days after the treatment(P<0.05). During the treatment, 3 cases of neck hematoma and 2 cases of nerve injury occurred in the conventional group, and no complications occurred in the experimental group. DISCUSSION The effect of ultrasound-guided phrenic nerve block in the treatment of intractable hiccup was satisfactory.
