卵巢上皮癌患者CA125和HE4水平及其临床意义分析

摘要
目的观察卵巢上皮癌患者的糖类抗原125(CA125)和人附睾蛋白4(HE4)表达,并分析其临床意义。方法选取2016年2月至2017年8月在该院接受治疗的卵巢上皮癌患者100例(卵巢癌组)为观察对象,同时选取同期在该院治疗的卵巢良性肿瘤患者为对照组。观察两组CA125和HE4表达,比较不同临床特征卵巢上皮癌患者CA125和HE4表达的差异,分析影响卵巢上皮癌患者CA125和HE4表达的因素。结果卵巢癌患者的HE4和CA125水平分别为(88.92±7.14)pmol/L、(96.47±5.03)U/mL,均高于对照组,差异有统计学意义(t=-89.783、-150.504,P<0.05);临床分期为Ⅲ+Ⅳ期、组织学分级为中、低分化、有淋巴结转移、死亡患者的HE4和CA125水平较高,不同年龄患者的HE4和CA125水平无明显差别;将单因素分析有意义的因素作为自变量,将HE4和CA125水平作为因变量进行多因素分析,结果显示,组织学分级和预后是影响卵巢癌患者血清HE4和CA125水平的因素。结论卵巢上皮癌患者的CA125和HE4表达水平较高,且受患者组织学分级和预后的影响。
Objective To observe the expression of carbohydrate antigen 125(CA125)and human epididymal protein 4(HE4)in patients with epithelial ovarian cancer and to analyze their clinical significance.Methods 100 cases of ovarian epithelial carcinoma patients(ovarian cancer group)treated in a hospital from February2016 to August 2017 were selected as the observation objects,and patients with benign ovarian tumors treated in a hospital at the same time were selected as control group.The expression of CA125 and HE4 in ovarian epithelial cancer patients with different clinical characteristics were observed and compared.The factors influencing the expression of CA125 and HE4 in ovarian epithelial cancer patients were analyzed.Results The levels of HE4 and CA125 in patients with ovarian cancer were(88.92±7.14)pmol/L,(96.47±5.03)U/mL,respectively,higher than those in control group,the difference was statistically significant(t=-89.783,-150.504,P<0.05).The levels of HE4 and CA125 were higher in patients with stageⅢ+Ⅳ clinical stage,middle or low histological grade,lymph node metastasis and death.There was no significant difference in the levels of HE4 and CA125 among patients of different ages.The significant factors of univariate analysis were taken as independent variables,and the levels of HE4 and CA125 were taken as dependent variables for multivariate analysis.The results showed that histological grade and prognosis were the factors influencing the levels of serum HE4 and CA125 in patients with ovarian cancer.Conclusion The expression levels of CA125 and HE4 in patients with epithelial ovarian cancer are higher,which are influenced by histological grade and prognosis.
