摘要
目的探讨Nrf2在甲状腺乳头状癌中的表达及与淋巴结转移的关系。方法应用免疫组化检测61例甲状腺乳头状癌中Nrf2的表达。结果 Nrf2总阳性率91.8%,其中无淋巴结转移组阳性率97.4%(38/39),有淋巴结转移组阳性率81.8%(18/22)(P<0.01);未侵犯包膜组阳性率100%(24/24),侵犯包膜组阳性率86.5%(32/37)(P<0.01);提示Nrf2在甲状腺乳头状癌中的表达与淋巴结转移及包膜侵犯呈负相关关系。结果还显示,在Nrf2阳性的病例中,未侵犯包膜与侵犯包膜组淋巴结转移率分别是16.7%(4/24)及43.8%(14/32)(P<0.05),提示尽管Nrf2阳性,侵犯包膜组明显比未侵犯包膜组淋巴结转移机会大;在侵犯包膜的病例中,Nrf2阴性及Nrf2阳性病例的淋巴结转移率分别是80%(4/5)及43.8%(14/32)(P<0.05),提示侵犯包膜即使Nrf2阴性时,淋巴结转移的可能性仍很大。结论 Nrf2与甲状腺乳头状癌侵袭转移密切相关,Nrf2阳性率降低能抑制肿瘤侵袭转移。
        Objective To explore the expression of Nrf2 in human thyroid papillary carcinoma and its relationship to lymph node metastasis. Methods The 61 cases surgical specimens of human thyroid papillary carcinoma were used to detect the expression of Nrf2 by immunohistochemistry. Results The positive rate of Nrf2 in all groups was 91.8%. The positive rate of no lymph node metastasis group was 97.4%(38/39). The positive rate of lymph node metastasis group was 81.8%(18/22). There was statistically different between no lymph node metastasis group and lymph node metastasis group in positive rate(P<0.01). The positive rate of no invading capsule group was 100%(24/24). The positive rate of capsule-invaded group was 86.5%(32/37). There was statistically different between non-capsule-invaded group and capsule-invaded group in positive rate(P<0.01). The expression of Nrf2 in human thyroid papillary carcinoma was negatively correlated with lymph node metastasis and capsular invasion. The results also showed that in the case of Nrf2 positive expression, the lymph node metastasis rate of the non-capsule-invaded group and the capsule-invaded group were 16.7%(4/24) and 43.8%(14/32). There was statistical difference between non-capsule-invaded envelope group and capsule-invaded group in lymph node metastasis rate(P<0.05). It was suggested that despite Nrf2 positive expression, the capsule-invaded group had more chance of lymph node metastasis than non-capsule-invaded group. In the case of capsule invasion, the lymph node metastasis rate of Nrf2 negative expression and Nrf2 positive expression were 80%(4/5)and 43.8%(14/32). There was statistically different between Nrf2 positive expression group and Nrf2 negative expression group in lymph node metastasis rate(P<0.05). It was suggested that despite capsule invasion, Nrf2 negative expression group had more chance of lymph node metastasis than Nrf2 positive expression group. Conclusion Nrf2 is closely related to the invasion and metastasis of thyroid papillary carcinoma. Decreased expression of Nrf2 can inhibit invasion and lymph node metastasis in thyroid papillary carcinoma.
