免疫型肠内营养对结肠癌根治术患者蛋白及炎性因子的影响

摘要
[目的]探讨免疫型肠内营养对结肠癌根治术患者蛋白及炎性因子的影响。[方法]选取2015年8月~2017年6月于我院接受根治术的结直肠癌患者96例,按照随机数字表法分为对照组和观察组,2组各48例,对照组患者给予常规肠内营养治疗,观察组患者接受免疫型肠内营养治疗,比较2组患者蛋白、炎性因子水平及不良反应发生率。[结果]2组患者术后第1天,白蛋白(ALB)、前白蛋白(PA)、免疫球蛋白G(IgG)水平较术前明显下降(P<0.05),术后第7天,观察组患者ALB、PA、IgG水平较术后第1天明显升高(P<0.05),对照组患者PA、IgG水平较较术后第1天明显升高(P<0.05),且观察组患者术后第7天ALB、PA、IgG水平明显高于对照组(P<0.05);2组患者术后第1天、第7天C反应蛋白(CRP)、白细胞介素6(IL-6)水平较术前均明显升高(P<0.05),术后第7天,CRP、IL-6水平较术后第1天明显降低(P<0.05),且观察组患者术后第7天CRP、IL-6水平明显低于对照组(P<0.05);观察组患者不良反应发生率明显低于对照组(P<0.05)。[结论]免疫型肠内营养可有效恢复结直肠癌根治术患者术后蛋白水平,减轻炎症反应,降低不良反应发生率,值得在临床上推广应用。
        [Objective]To investigate the effect of immune-type enteral nutrition on protein and inflammatory factors in patients with colon cancer.[Methods]A total of 96 patients with colorectal cancer undergoing radical operation in our hospital from Aug.2015 to Jun.2017 were divided into control group(48 cases)and observation group(48 cases)according to the random number table method,the patients in control group were given conventional enteral nutrition treatment,the patients in the observation group received immunological enteral nutrition treatment,the protein,inflammatory cytokines and the incidence of adverse reactions in the two groups were compared.[Results]1 d after operation,the levels of albumin(ALB),prealbumin(PA),immunoglobulin G(IgG)in the two groups were significantly lower than those before operation(P<0.05),7 d after operation,the levels of ALB,PA and IgG in the observation group were significantly higher than those at 1 d after operation(P<0.05),the levels of PA and IgG in the control group were significantly higher than those at 1 d after operation(P<0.05),The levels of ALB,PA and IgG in the observation group were significantly higher than those in the control group at 7 d after operation(P<0.05);1 d and 7 d after operation,the levels of C reactive protein(CRP),interleukin 6(IL-6)were significantly higher than those before operation(P<0.05),and the levels of CRP and IL-6 at 7 d after operation were significantly lower than those at 1 d after operation(P<0.05),the levels of CRP and IL-6 in the observation group at 7 d after operation were significantly lower than those in the control group(P<0.05);The incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).[Conclusion]The immune-type enteral nutrition can effectively restore the postoperative protein levels,reduce the inflammatory reaction and reduce the incidence of adverse reactions in patients with colorectal cancer.It is worth to be popularized and applied clinically.
