摘要
目的研究二维斑点追踪成像(2D-STI)与实时三维超声心动图(RT-3DE)评价不同部位起搏器植入术后左心室收缩功能与同步性。方法选取2017年1月至2017年12月我院收治的永久性心脏双腔起搏器植入术患者60例,根据起搏器植入部位不同分为RVA组(右心室心尖部起搏)及RVOT组(右心室流出道起搏)各30例。比较两组患者手术前后的2D-STI、RT-3DE的左心室收缩功能。结果术前,两组的2D-STI、 RT-3DE左心室基底段LS、中间段LS、心尖段LS比较均无统计学差异(P>0.05);术后, RVA组的心尖段LS明显高于术前(P <0.05)。两组患者术后的左心室收缩功能低于术前, RVA组与RVOT组术后均会引起左心室收缩不一致,其中RVOT组的左心室收缩功能优于RVA组。结论 2D-STI与RT-3DE均能够评价左心室收缩同步性,值得临床推广应用。
        Objective To study the left ventricular systolic function and synchronism after different pacemaker implantation evaluated by 2D speckle tracking imaging(2D-STI) and real-time 3D echocardiography(RT-3DE). Methods 60 cases of patients with permanent cardiac dual-chamber pacemaker implantation admitted to our hospital from January 2017 to December 2017 were selected and randomly divided into RVA group(right ventricular apex pacemaking) and RVOT group(right ventricular outflow tract pacemaking)according to different pacemaker implantation sites. The left ventricular systolic function of 2D-STI and RT-3DE before and after surgery were compared between the two groups. Results Before surgery, no statistical difference was found between the two groups in the left ventricular basal segment LS, central segment LS and apical segment of 2D-STI and RT-3DE(P >0.05). After surgery, the apical segment LS of the RVA group was significantly higher than that before surgery(P <0.05). The postoperative left ventricular systolic function in both groups were significantly lower than those before surgery, the postoperative left ventricular systole in both RVA group and RVOT group were inconsistent, and the left ventricular systolic function in RVOT group was superior to that in RVA group. Conclusions Both 2D-STI and RT-3DE can evaluate left ventricular systolic synchronism, which is worthy of clinical promotion and application.
