小剂量胰岛素联合电解质补充治疗儿童糖尿病酮症酸中毒的临床价值

摘要
目的评价在儿童糖尿病酮症酸中毒治疗中采用小剂量胰岛素与电解质补充联合治疗的临床治疗效果。方法该研究选择的样本病例为该院收治的儿童糖尿病酮症酸中毒患儿40例,以2011年10月—2016年10月为样本选取时段,所有患儿均予以小剂量胰岛素与电解质补充联合治疗,对患儿治疗前后血清炎症因子变化、血钾、血钠、血糖进行比对和分析,并对患儿酸中毒纠正时间及住院时间进行记录。结果经小剂量胰岛素与电解质补充联合治疗,患儿酸中毒纠正时间为(13.48±1.11)d;治疗后患儿血钾水平(4.69±0.76)mmol/L,血糖水平(12.06±2.86)mmol/L,血钠水平为(142.70±9.95)mmol/L均比治疗前明显较优,治疗前后对比差异有统计学意义(P<0.05);同时治疗前后患儿血清炎症因子(TNF-α、IL-18、IL-12、IL-6)比对差异有统计学意义(P<0.05);患儿住院时间为(14.01±1.03)d。结论在儿童糖尿病酮症酸中毒治疗中采用小剂量胰岛素与电解质补充联合治疗具有确切的治疗效果,不但可对患儿高血酮及高血糖进行明显改善,同时具有有效性和安全性,值得应用和推广。
        Objective To evaluate the clinical value of small-dose insulin and Electrolyte supplement therapy of children with diabetic ketoacidosis. Methods 40 cases of children with diabetic ketoacidosis admitted and treated in our hospital were selected from October 2011 to October 2016 and all children were treated with small-dose insulin and electrolyte supplement therapy, and the serum inflammatory factor changes, blood potassium, blood sodium and blood glucose of children before and after treatment were compared and analyzed and the correction time and length of stay of children were recorded.Results After treatment, the correction time was(13.48±1.11)d, and the blood potassium, blood glucose and blood sodium levels of children after treatment were respectively(4.69 ±0.76)mmol/L,(12.06 ±2.86)mmol/L and(142.70 ±9.95)mmol/L,which were obviously better than those before treatment, and the differences were obvious with statistical significance(P <0.05), and the differences in the TNF-α,IL-18,IL-12 and IL-6 before and after treatment were obvious with statistical significance(P<0.05), and the length of stay was(14.01±1.03)d. Conclusion The effect of small-dose insulin and electrolyte supplement therapy of children with diabetic ketoacidosis is definite, which can obviously improve the hyperglycemia and high blood glucose, and it is effective and safe, which can be applied and promoted.
