某院住院药房不合理用药医嘱干预记录分析

摘要
目的分析某院住院药房不合理用药医嘱干预记录情况,为临床安全、有效、合理用药提供参考。方法采用回顾性分析,对某院2016年1月～2018年6月住院药房不合理用药医嘱干预记录情况进行统计分析。结果共记录不合理用药医嘱303条,干预后被采纳医嘱250条,占82.5%。不合理用药医嘱类型主要包括用法用量不合理、医嘱录入错误、重复给药、联合用药不合理、药物配伍不合理、药物浓度不合理、溶剂选择不合理、给药途径不合理。结论药师对医嘱进行审核和干预,可提高住院患者用药的安全性、有效性及合理性。
Objective To analysis of unreasonable medication order intervention record in a hospital pharmacy and to provide reference for clinical safe, effective and rational drug use. Methods The retrospective analysis method was adopted to conduct a statistical analysis of the records of unreasonable medical order intervention in the inpatient pharmacy of the hospital from January 2016 to June 2018. Results A total of 303 unreasonable medical orders were recorded, and 250 were adopted after intervention, accounting for 82.5%.The types of unreasonable medical orders mainly include unreasonable usage and dosage, incorrect input of medical orders, repeated administration, unreasonable combination administration, unreasonable compatibility of medicine,unreasonable concentration of medicine, unreasonable solvent selection and unreasonable way of administration. Conclusion The examination and intervention of doctor's advice by pharmacists can improve the safety, effectiveness and rationality of the medication for inpatients.
