止痒方熏洗结合穴位自血疗法治疗慢性肛周湿疹65例

摘要
目的探讨止痒方熏洗结合穴位自血疗法治疗慢性肛周湿疹的临床疗效。方法将195例慢性肛周湿疹患者采用随机数字表分为试验组、对照组1及对照组2,每组65例。对照组1予长强穴普通针刺配合止痒方熏洗治疗;对照组2予单独使用止痒方熏洗治疗;试验组采用自血疗法配合止痒方熏洗治疗。治疗周期均为2个疗程,共20 d。结果试验组、对照组1及对照组2治疗后总有效率分别为93. 8%、80%、73. 8%,试验组总有效率明显高于对照组1、对照组2(P <0. 05),差异均具有统计学意义。经治疗2个疗程治疗后,试验组症状积分明显低于对照组1及对照组2。治疗结束3年后随访有效病例,试验组、对照组1及对照组2复发率分别为13. 85%、43. 08%、70. 8%,试验组复发率明显低于对照组1及对照组2(P <0. 05),差异均具有统计学意义。结论采用止痒方熏洗结合穴位自血疗法治疗慢性肛周湿疹临床疗效确切,能有效控制复发率,值得临床推广。
Objective To explore the clinical effects on chronic perianal eczema treated with the fumigation with zhiyang formula and autohemotherapy at acupoint. Methods A total of 195 patients of chronic perianal eczema were divided into a trial group,a control No. 1 group and a control No. 2 group,65 cases in each one according to random number table. In the control No. 1 group,the cases were treated with acupuncture at Changqiang(GV 1) and fumigation with zhiyang formula. In the control No. 2 group,the fumigation with zhiyang formula was used singly. In the trial group,the autohemotherapy was used in combination with fumigation with zhiyang formula. The duration of treatment was 2 courses,for 20 days in total. Results The total effective rate was 93. 8%,80% and 73. 8% in the trial group,the control No. 1 group and the control No. 2 group successively. The total effective rate in the trial group was higher obviously than the control No. 1 and No. 2 groups,indicating the significant differences(P < 0. 05). After 2-course of treatment,the symptom score in the trial group was lower obviously than the control No. 1 and No. 2 groups separately. In the follow-up visit on the effective cases3 years after the end of treatment,the recurrence rate was 13. 95%,43. 08% and 70. 8% in the trial group,the control No.1 group and the control No. 2 group successively. The recurrence rate in the trial group was lower obviously than the control No. 1 and No. 2 groups,indicating the significant differences(P < 0. 05). Conclusion The fumigation with zhiyang formula together with autohemotherapy at acupoint achieves the definite clinical effects on chronic perianal eczema. Such combined therapy effectively controls the recurrence rate,thus it deserves to be promoted in clinical practice.
