个体化Bryant牵引带对婴幼儿股骨干骨折皮肤的影响研究

摘要
目的探讨个体化Bryant牵引带对婴幼儿股骨干骨折皮肤的影响。方法选取2015年1月—2018年5月婴幼儿股骨干骨折住院治疗患儿120例,分为对照组60例和观察组60例,对照组采用胶布+普通弹力绷带,观察组采用个体化量体订制内衬海绵胶布+自粘性弹力绷带。观察并对比两组患儿牵引带松动脱落次数、皮肤损伤程度。结果观察组中60例患儿采用个体化订制Bryant牵引带脱落4例(6.7%),皮肤完好59例(98.3%),皮肤过敏1例(1.6%),压疮1例(1.6%),皮肤感染1例(1.6%);对照组中60例患儿牵引带脱落18例(30.0),皮肤完好45例(70.0%),皮肤过敏15例(25.0%),压疮9例(15.0%),皮肤感染10例(16.7%)。两组牵引带脱落次数及皮肤并发症比较(χ~2=10.91、14.13、14.59、8.57、7.43,P<0.01)。结论个体化Bryant牵引带具有固定稳定,不易滑脱,不易损伤皮肤,松紧适宜、舒适性强等优点。在治疗婴幼儿股骨干骨折中疗效突出并发症较少。
Objective To study the effect of individualized Bryant traction belt on the skin of femoral shaft fracture in infants. Methods 120 cases of infants with skin of femoral shaft fracture in our hospital from January 2015 to May 2018 were selected and divided into two groups with 60 cases in each, the control group used the rubber and ordinary elastic bandage, while the observation group used the individualized volume customization of lined sponge tape and self-adhesive elastic bandage, and the number of loosening and falling off of traction belt and skin injury degree were observed and compared. Results Of 60 cases, 4 cases(6.7%) had individual-customized Bryant traction belt shedding,59 cases(98.3%) had skin intact, 1 cases(1.6%) had skin sensibility, 1 cases(1.6%) had the pressure cores, 1 cases(1.6%) had the skin infection; in the control group, the corresponding numbers were respectively 18 cases(30.0%), 45 cases(70.0%), 15 cases(25.0%), 9 cases(15.0%), 10 cases(16.7%),the difference was statistically significant(χ~2= 10.91,14.13, 14.59, 8.57, 7.43, P<0.01). Conclusion The individualized Bryant traction belt is fixed and steady, and it is difficult to slip and damage the skin, with proper tension and strong comfort, and the curative effect in treatment of skin of femoral shaft fracture in infants is outstanding, with few complications.
