全冠修复治疗不同时期牙隐裂的疗效评价

摘要
目的对全冠修复治疗不同时期牙隐裂的临床效果进行评价。方法选取48例2015年1月～2017年1月在我院治疗的牙隐裂患者,本组患者共有患牙48颗,根据本组患者牙髓的状态将其分为早期组(25例,浅隐裂无症状患者或者是隐裂较深,只对冷和热有明显的刺激感,咀嚼不适,但是没有牙髓炎、牙周炎等症状的隐裂牙)和晚期组(23例,伴有牙髓炎以及牙周炎等症状的隐裂牙),早期组患者给予局部麻醉牙体预备后,采用金属烤瓷牙全冠修复,晚期组患者先进行根管治疗,再采用金属烤瓷牙全冠修复。对本组患者随访2年后,观察本组患者的临床治疗效果。结果早期组患者半年、1年、2年的成功率分别为84.00%、76.00%、72.00%;晚期组患者半年、1年、2年的成功率分别为95.65%、86.96%、78.26%。本组48例患者经过治疗后,2年后的随访结果显示,治疗成功43颗牙,成功率达89.58%,失败5颗牙,占比10.42%。在失败的5颗牙中,有2颗牙在经过治疗后,反复的出现了咬合疼痛,患者自愿要求拔除,有2颗牙出现了牙体的纵裂松动而拔除,有1颗牙在经过修复后出现了牙折裂而拔除。结论在治疗牙隐裂患者时,采用全冠修复治疗,不但能够完全修复患牙与上牙颌之间的关系,并且对保护患者,避免其发生折裂等均有重要的意义,是目前临床上治疗牙隐裂的一种理想的方法,有着较高的推广价值。
Objective To evaluate the clinical effect of crown restoration in the treatment of cracks in different periods. Methods A total of 48 patients with cracked teeth in our hospital from January 2015 to January 2017 were enrolled.According to the status of dental pulp of the patients,the patients were divided into the early group(25 patients with no symptoms of superficial cleft or deep cleft,with obvious irritation to cold and heat and chewing discomfort,but no symptoms of pulpitis and periodontitis)and the late group(23 patients with symptoms of pulpitis and periodontitis).The patients in the early stage group were treated with partial anesthesia and complete crown restoration with metal porcelain,while the patients in the advanced group were treated with root canal first,and then repaired with metal porcelain crowns.Patients were followed up for 2 years to observe the clinical treatment effect. Results The success rates of the early group were 84.00%,76.00%,and 72.00% for the first half,1 year and 2 years,respectively,while the success rates for the late group of patients were 95.65%,86.96%,and 78.26% for half year,1 year,and 2 years,respectively.After the treatment of 48 patients in this group,the follow-up results after 2 years showed that there were 43 teeth treated successfully,with success rate of 89.58%,and there were 5 teeth treated failed,accounting for10.42%.Among the 5 teeth which failed,2 teeth repeatedly suffered from occlusal pain after treatment,and patients voluntarily requested removal;2 teeth were pulled out due to loose longitudinal fissure of the tooth,and 1 tooth was pulled out due to fracture after repair. Conclusion In the treatment of patients with cracked teeth,the use of crown repair treatment can not only completely repair the relationship between the teeth and the upper jaw,but also have important significance for protecting patients and avoiding cracking.An ideal method for the treatment of cracked teeth in clinical practice has a high promotion value.
