腹腔镜手术与传统开腹手术在妇科术后粘连性肠梗阻中的综合疗效对比

摘要
目的比较腹腔镜手术与传统开腹手术在妇科术后粘连性肠梗阻中的综合疗效。方法将60例妇科术后粘连性肠梗阻患者分两组进行治疗,其中30例采取腹腔镜手术,另外30例采取传统开腹手术,分别设为腹腔镜组与开腹组。结果两组患者手术时间差异无统计学意义(P>0.05),腹腔镜组在切口长度、术中出血量、术后进食时间及住院时间方面均优于开腹组,差异显著(P<0.01);两组患者的肠梗阻复发情况差异无统计学意义(P>0.05),腹腔镜组的并发症总发生率为13.33%,开腹组为36.67%,差异具有统计学意义(P<0.01);腹腔镜组的术后睡眠质量优于开腹组,差异显著(P<0.01)。结论与传统开腹手术相比,腹腔镜手术在妇科术后粘连性肠梗阻的治疗方面体现出了更多的优势,极具临床应用价值。
Objective To compare the effect of laparoscopic operation and traditional open surgery in the treatment of postoperative adhesion intestinal obstruction. Methods Sixty patients with adhesive intestinal obstruction after gynecological operation were selected and devided into two groups. The 30 patients were treated with laparoscopic operation as laparoscopic group and the others were treated with traditional open surgery as open group. Results The operation time of the two groups showed no significant difference(P >0.05), the incision length, bleeding volume,postoperative feeding time and length of stay of the laparoscopic group were better than those of the control group, and the differences were significant(P <0.01). There was no significant difference in the recurrence of intestinal obstruction between the two groups(P>0.05). The total complication rate was 13.33% in the laparoscopic group and 36.67% in the open group(P <0.01). In the laparoscopic group, the sleep quality was better than that of the open group the difference between the two group was significant, and the difference was significant(P<0.01). Conclusion Compared with traditional open surgery, laparoscopic surgery in the treatment of postoperative intestinal obstruction in patients with adhesive intestinal obstruction has more advantages, which has great value in clinical application.
