肌内效贴联合等速肌力训练治疗乳腺癌术后淋巴水肿的疗效

摘要
目的探讨肌内效贴联合等速肌力训练治疗乳腺癌术后淋巴水肿的临床疗效。方法选取2016年1月至2018年1月间江苏省常州市妇幼保健院收治的60例乳腺癌术后并发淋巴水肿患者,采用随机数表法分为观察组和对照组,每组30例。观察组患者采用在常规康复训练基础上引入肌内效贴联合等速肌力训练治疗,对照组患者采用常规康复训练治疗。观察并比较两组患者治疗前后的上肢功能评定量表(DASH)、双侧上臂周径差值、上肢淋巴流量及肩关节活动度。结果治疗后,观察组DASH评分明显低于对照组,差异有统计学意义(P <0. 05)。观察组双侧上臂周径差值明显低于对照组,而观察组上肢淋巴流量则明显高于对照组,差异均有统计学意义(均P <0. 05)。观察组前屈、后伸、内收及外展等肩关节活动度均明显高于对照组,差异均有统计学意义(均P <0. 05)。结论肌内效贴配合等速肌力训练可增加乳腺癌患者术后患侧上肢淋巴回流量,有效减少淋巴水肿,提高上肢功能及肩关节活动度,值得临床推荐。
Objective To observe the clinical efficacy of kinesio tape combined with isokinetic muscle training for lymphedema in post-operative patients with breast cancer. Methods Sixty patients with postoperative lymphedema admitted to Changzhou Maternal and Child Health Hospital from January 2016 to January 2018 were randomly selected and divided into an observation group and a control group with 30 patients in each group. The observation group was given kinesio tape combined with isokinetic muscle training besides routine rehabilitation training. The control group was only given routine rehabilitation training.The upper extremity function rating scale disability of arm,shoulder and hand( DASH),the difference in the diameter of the bilateral upper arms,the upper extremity lymph flow and the shoulder joint activity were observed and compare d between the two groups before and after the treatment. Results After the treatment,the DASH score was significantly lower in the observation group than in the control group( P <0. 05). The difference in the circumference of the upper arms was significantly lower in the observation group than in the control group,and the lymph flow in the upper arm was significantly higher in the observation group than in the control group( all P < 0. 05). The shoulder activities including flexion,extension,adduction and abduction were significantly higher in the observation group than in the control group( P <0. 05). Conclusion Kinesio tape combined with isokinetic muscle training can increase the lymphatic recirculation in affected upper limbs after breast cancer surgery,effectively reduce lymphedema and improve upper limb function. It is worthy of clinical recommendation.
