全麻、硬膜外麻醉对高龄全髋关节置换术患者麻醉效果、凝血功能的影响比较

摘要
目的分析在全髋关节置换术中全麻、硬膜外麻醉两种不同麻醉方法对患者麻醉、凝血功能的影响。方法选取我院在2017年5月至2018年5月收治的患者120例作为研究对象,依据随机的原则、按照就诊顺序分成实验组(硬膜外麻醉)和对照组(全麻),每组60例,比较分析两组结果的差异性。结果实验组患者麻醉24 h后效果优于对照组,72 h后差别不大,麻醉后6 h以及24 h凝血指标有明显差异;麻醉90 min后血小板聚集率明显较术前下降,并在麻醉后3 d恢复到正常水平。结论硬膜外麻醉具有较好的麻醉效果,同时对患者凝血功能以及认知功能影响较小,安全性较高。
Objective To analyze the effects of two different anesthesia methods,general anesthesia and epidural anesthesia,on anesthesia and coagulation function in patients undergoing total hip arthroplasty.Methods A total of 120 patients admitted to our hospital from May 2017 to May 2018 were enrolled in the study.According to the principle of randomization,they were divided into experimental group(epidural anesthesia)and control group(general anesthesia)according to the order of treatment.In 60 cases,the differences between the two groups were compared and analyzed.Results The anesthesia of the experimental group was better than that of the control group after anesthesia for 24 hours.There was no significant difference after 72 hours.There were significant differences in coagulation parameters between 6 h and 24 h after anesthesia.Conclusion Epidural anesthesia has a good anesthetic effect,and it has little effect on the coagulation function and cognitive function of patients,and it has higher safety.
