高压氧辅助标准大骨瓣开颅术对重型颅脑损伤病人血流动力学及血清CRP的影响

摘要
目的探讨高压氧辅助标准大骨瓣开颅术对重型颅脑损伤病人血流动力学及血清C反应蛋白的影响。方法选择2000年1月—2017年1月我院神经外科、重症监护室(ICU)、急诊及在外院已行大骨瓣开颅术术后转入高压氧科的重型颅脑损伤病人112例作为研究对象,根据治疗方法不同分为研究组与对照组,各56例,对照组给予标准大骨瓣开颅术治疗,研究组在对照组基础上给予高压氧辅助治疗,以10 d为1个疗程,观察治疗3个疗程,记录并比较血流动力学及血清炎性因子变化。结果治疗后,两组格拉斯哥昏迷量表评分(GCS)显著高于治疗前(P<0.05),且研究组GCS评分显著高于对照组(P<0.05)。研究组治疗期间脑疝、下肢静脉血栓等并发症发生率为10.7%,显著低于对照组的32.1%(P<0.05)。治疗后,两组C反应蛋白水平显著低于治疗前(P<0.05),且研究组低于对照组(P<0.05)。治疗后,研究组大脑中动脉收缩末期峰值流速(Vp)和舒张期末期峰值流速(Vd)显著高于治疗前(P<0.05),且研究组Vp与Vd显著高于对照组(P<0.05)。结论高压氧辅助标准大骨瓣开颅术治疗重型颅脑损伤能减轻病人炎性因子的表达,减少继发性并发症的发生,改善颅内动脉血流动力学,从而起到显著脑保护作用,促进病人的预后康复。
