血清胱抑素c在老年高血压患者早期肾损害中的诊断价值分析

摘要
目的分析血清胱抑素c(Cyst C)在老年高血压患者早期肾损害中的诊断价值。方法选取2016年1月-2017年12月我院收治120例确诊为老年高血压患者,根据有无蛋白尿分为A组(微量蛋白尿组,n=62),B组(无蛋白尿组,n=58),并选择60名健康体检人员(对照组,n=60)作为研究对象,分别测量以上三组人群的收缩血压(SBP)、舒张血压(DBP)、血清Cyst C、血肌酐(Scr)、血尿素氮(BUN)、肾小球滤过率(GFR)等指标并进行统计学分析。结果 A组与B组患者SBP、DBP明显高于对照组(P<0.05);A组mALB、CysC水平明显高于B组,GFR明显低于A组(P<0.05);A组Scr水平高于B组与对照组,但差异无统计学意义(P>0.05)。结论与传统诊断指标相比,CysC能够较为灵敏的反映患者肾功能的早期损伤,有可能成为老年高血压肾病早期诊断的指标,值得推广。
Objective To analyze the diagnostic value of serum cystatin C(Cyst C) in early stage renal impairment in elderly hypertensive patients. Methods From January 2016 to December 2017, 120 elderly patients with hypertension diagnosed in our hospital were selected and divided into group A(microalbuminuria group, n=62) according to whether there was proteinuria or not, group B(no protein In the urine group(n=58), 60 healthy persons(control group,n=60) were selected as the study subjects. The systolic blood pressure(SBP), diastolic blood pressure(DBP), serum Cyst C, blood were measured in the above four groups. Creatinine(Scr), blood urea nitrogen(BUN), glomerular ?ltration rate(GFR) and other indicators and statistical analysis. Results The SBP and DBP in group A and B were signi?cantly higher than those in control group(P<0.05). The levels of mALB and CysC in group A were signi?cantly higher than those in group B, and the GFR was signi?cantly lower than that in group A(P<0.05). Obvious B group and control group, but the difference was not statistically signi?cant(P>0.05). Conclusion Compared with traditional diagnostic indicators, CysC can respond sensitively to early renal injury in patients, and may be an indicator for early diagnosis of elderly hypertensive nephropathy. It is worthy of promotion.
