活血化瘀法治疗背景型糖尿病视网膜病变的疗效观察

摘要
目的观察中医活血化瘀法治疗背景型糖尿病视网膜病变疗效。方法 30例(60患眼)背景型糖尿病视网膜病变患者,按照治疗时间不同分为对照组(2015年6月~2016年3月)与观察组(2016年4月~2017年7月),每组15例(30患眼)。对照组给予食疗以及西药常规降糖对症治疗,观察组在对照组基础上待血糖控制平稳的情况下给予活血化瘀为主的中医治疗,比较两组患者的治疗效果,治疗前后荧光素眼底血管造影检测结果、血管内皮炎性因子水平以及血液流变学指标。结果观察组患者治疗总有效率为93.33%(28/30),明显高于对照组的63.33%(19/30),差异有统计学意义(P<0.05)。治疗后,观察组患者微血管瘤数量、出血灶面积、渗出灶面积、黄斑水肿面积明显优于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者血管内皮炎性因子水平明显优于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者血液流变学指标明显优于对照组,差异有统计学意义(P<0.05)。结论中医活血化瘀可以有效提高背景型糖尿病视网膜病变治疗效果,值得临床推广。
Objective To observe the efficacy of traditional Chinese medicine of Huoxue Huayu therapy in treating background diabetic retinopathy. Methods A total of 30(60 involved eyes) patients with background diabetic retinopathy were divided control group(June 2015~ March 2016) and observation group(April 2016 ~ July 2017) into by different treatment time, with 15 cases(30 involved eyes) in each group. The control group received dietotherapy and Western medicine routine hypoglycemic therapy, and the observation group received traditional Chinese medicine treatment of Huoxue Huayu therapy when the blood sugar was controlled smoothly on the basis of the control group. Comparison were made on treatment effect, results of fundus fluorescein angiography before and after treatment, the level of vascular dermatitis and hemorrheology indicators between the two groups. Results The observation group had obviously higher total treatment effective rate as 93.33%(28/30) than 63.33%(19/30) in the control group, and the difference was statistically significant(P<0.05). After treatment, the observation group had obviously better number of microangiomas, hemorrhage area, exudate area, macular edema area than the control group, and the differences were statistically significant(P<0.05). After treatment, the observation group had obviously better level of vascular dermatitis than the control group, and the difference was statistically significant(P<0.05). After treatment, the observation group had obviously better hemorrheology indicators than the control group, and the difference was statistically significant(P<0.05). Conclusion Traditional Chinese medicine treatment of Huoxue Huayu can effectively improve the treatment effect background of diabetic retinopathy, and it is worthy of clinical promotion.
