摘要
目的探讨中孕期彩色多普勒超声检测对胎儿先天性心脏病的诊断价值。方法选取2014年5月—2017年5月收治的2494例中孕期孕妇作为研究对象。所有孕妇均经常规超声以及彩色多普勒超声检查,且产后新生儿均进行超声检查,引产后胎儿均进行尸检。比较不同检查方法与产后新生儿或引产后胎儿确诊结果的差异。结果本组共确诊为胎儿先天性心脏病14例,发病率为0. 56%。常规超声对中孕期胎儿先天性心脏疾病的诊断率与确诊结果比较差异具有统计学意义(χ2=4. 279,P=0. 039)。彩色多普勒超声对中孕期胎儿先天性心脏疾病的诊断率显著高于常规超声(χ2=3. 864,P=0. 042),且与确诊结果比较差异无统计学意义(χ2=0. 037,P=0. 847)。结论彩色多普勒超声检查对中孕期胎儿先天性心脏病具有较高检出率,且具有一定的安全性和便捷性。
        Objective To investigate value of color Doppler ultrasonography in diagnosis of fetal congenital heart disease in mid-gestation. Methods A total of 2494 pregnant women admitted during May 2014 and May 2017 were selected as research objects. All pregnant women received conventional examinations of ultrasound and color Doppler ultrasound,and postpartum neonates were examined by ultrasound,and the fetuses after induction of labor were given autopsy. Differences of pathological and anatomic results between different examination methods with confirmation results of postpartum neonates after induction of labor. Results In this group,14 fetuses were diagnosed as having congenital heart disease,and the incidence rate was 0. 56%. The differences in diagnosis rate by conventional ultrasonography with confirmed results in fetal congenital heart disease in mid-gestation were statistically significant(χ2= 4. 279,P = 0. 039). Diagnosis rate by color Doppler ultrasound was significantly higher than that by conventional ultrasonography in fetal congenital heart disease in mid-gestation(χ2= 3. 864,P = 0. 042),and there were no significant differences between the screening rates with confirmed results in two groups(χ2= 0. 037,P = 0. 847). Conclusion Color Doppler ultrasonography has a high detection rate in fetal congenital heart disease in mid-gestation with a certain safety and convenience.
