脐带扭转对新生儿阿氏评分的影响分析

摘要
目的探讨婴儿在母亲腹中发生脐带扭转对新生儿阿氏评分的影响分析。方法随机抽取2016年2月—2018年8月期间在该院生产的产妇70例作为临床观察对象。采用随机数字表法分成观察组(婴儿在母亲腹中发生脐带扭转,脐带扭转螺距值小于2.0及脐动脉血流S/D比值大于3.0,35例)和对照组(婴儿在母亲腹中未发生脐带扭转,35例)。对比分析两组产妇胎儿脐动脉血流参数、脐带扭转螺距值、阿氏评分及剖宫产率的结果分析。结果观察组新生儿阿氏评分≤7分(28.57%),发生胎儿宫内窘迫(37.14%),孕妇剖宫产(40.00%)与对照组新生儿阿氏评分小于等于7分(11.43%),发生胎儿宫内窘迫(17.14%),孕妇剖宫产(14.29%),比较差异均有统计学意义(χ~2=5.356,5.321,5.394,P<0.05)。结论作为评估产妇生产胎儿的危险度重要指标,脐带扭转螺距值及脐动脉血流参数的结果具有重要价值。当婴儿在母亲腹中发生脐带扭转(脐带扭转螺距值小于2.0、脐动脉血流S/D比值大于3.0时),胎儿发生宫内窘迫发生的概率被明显加大,新生儿阿氏评分小于等于7分的概率被显著加大,产妇剖宫产率也被明显提高,具有较高的临床价值。
Objective To investigate the influence of umbilical cord torsion in the mother's abdomen on neonatal Alzheimer's score. Methods Seventy parturients who delivered in our hospital from February 2016 to August 2018 were randomly selected for clinical observation. Umbilical cord torsion occurred in the mother's abdomen in infants, the value of umbilical cord torsion pitch was less than 2.0 and the ratio of S/D of umbilical artery blood flow was more than 3.0 in 35 cases. The control group(35 cases, 35 cases, no umbilical cord torsion occurred in the mother's abdomen). The umbilical artery blood flow parameters, umbilical cord torsion pitch value, Aldrich score and cesarean section rate were compared and analyzed between the two groups. Results In the observation group, neonatal Alzheimer's score was less than or equal to 7 points(28.57%), fetal distress(37.14%), cesarean section(40.00%) and control group, neonatal Alzheimer's score was less than or equal to 7 points(11.43%), fetal distress(17.14%) and cesarean section(14.29%) occurred in pregnant women.Meaning(χ~2=5.356,5.321,5.394, P < 0.05). Conclusion As an important index to evaluate the risk of maternal fetal delivery,the value of umbilical cord twist pitch and umbilical artery blood flow parameters have important value. When the umbilical cord torsion occurs in the mother's abdomen(the umbilical cord torsion pitch value is less than 2.0, the umbilical artery blood flow S/D ratio is greater than 3.0), the incidence of fetal distress is significantly increased, the probability of neonatal Alzheimer's score is less than or equal to 7 points is significantly increased, the cesarean section rate is also significantly increased, with a higher impending.
