血清CEA、CA199、CA242、CA724检测对胃癌的诊断及预后评估的价值探讨

摘要
目的探究血清CEA、CA199、CA242、CA724检测在胃癌诊断及预后评估中的应用价值。方法将2012年1月—2016年6月该院收治的经病理活检确诊为胃癌的60例患者纳入观察组,另将同期收治的55例良性胃病患者纳入对照A组,58名健康体检者纳入对照B组,均进行血清CEA、CA199、CA242、CA724水平检测,分析其检测结果。结果观察组患者的血清CEA(31.02±5.41)ng/mL、CA199(66.56±10.41)IU/mL、CA242(31.54±3.08)IU/mL、CA724(18.89±7.08)IU/mL,均显著高于对照A组、对照B组(P<0.01)。且在观察组中,血清CEA、CA199、CA242、CA724水平会随病理分期的增高而增加(F=446.604、331.561、107.161、258.662,P<0.01)。随访2年发现,生存组患者的CEA(42.85±7.26)ng/mL、CA199(69.05±11.51)IU/mL、CA242(54.54±7.77)IU/mL、CA724(24.83±9.21)IU/mL,均显著低于死亡组(P<0.01)。结论血清CEA、CA199、CA242、CA724检测能够有效提高胃癌的诊断准确率,且其动态变化与胃癌患者病情发展及预后转归有密切的关系,可成为胃癌早期诊断和预后评估的重要辅助指标。
Objective To study the application value of serum CEA, CA199, CA242, CA724 test in diagnosis and prognosis evaluation of gastric cancer. Methods 60 cases of patients with gastric cancer confirmed by the pathological biopsy in our hospital from January 2012 to June 2016 were selected as the observation group, while 55 cases of patients with benign gastric disease admitted and treated at the same period were selected as the control A group, and 58 cases of healthy physical examination people were selected as the control B group, and the patients were for serum CEA,CA199, CA242, CA724 test, and the test results were analyzed. Results The serum CEA( 31.02±5.41) ng/mL, CA199(66.56±10.41) IU/mL, CA242( 31.54±3.08) IU/mL, CA724(18.89±7.08) IU/mL in the observation group were obviously higher than those in the control A group and in the control B group(P<0.01), with the increase of pathological staging,the serum CEA, CA199, CA242, CA724 levels increased in the observation group(F =446.604、331.561、107.161、258.662,P<0.01), and 2-year follow-up showed that the CEA(42.85±7.26) ng/mL, CA199(69.05±11.51) IU/mL, CA242(54.54±7.77) IU/mL, CA724( 24.83±9.21) IU/mL were obviously lower than those in the death group(P<0.01). Conclusion The serum CEA, CA199, CA242, CA724 test can effectively improve the diagnosis accurate rate of gastric cancer, and the dynamic changes have a close relationship with the disease progression and prognosis outcome of patients with gastric cancer, which can be used as an important adjunctive indicator of early diagnosis and prognosis evaluation of gastric cancer.
