基于病案首页信息的住院绩效考核优化研究

摘要
随着医疗卫生事业改革的不断深化,公立医院绩效考核问题不断凸显,绩效改革面临着严峻的挑战,建立科学、合理的绩效考核体系有利于实现医院管理的精细化,促进医院健康、可持续发展。本文重点阐述某院住院绩效改革过程中,充分利用病案首页信息优化绩效考核:首先,在科室内部以"主诊医师"为依据划分医师组,并将住院绩效考核细化到各医师组;第二,通过对病案首页数据的提取与分析,从工作量、工作质量、服务效率、医疗费用等多个维度选取考核指标,综合反映住院医疗运行情况;第三,每月利用住院病案数据测算考核结果,用于对科室当月进行绩效评价并兑现月度绩效,实现绩效考核与分配的有效衔接。通过不断优化住院绩效考核体系,促进科室进一步加强对疑难杂症的诊治,提高住院服务能力,规范病案首页的填写,使某院住院管理逐步实现精细化。
        With the continuous deepening of the reform of medical and health services, the problem of performance appraisal in public hospitals has become increasingly prominent, and the performance reform faces severe challenges. Establishing a scientific and reasonable performance appraisal system is conducive to the refinement of hospital management and the promotion of hospital health and sustainable development. This paper focuses on the optimization of performance evaluation by making full use of the information on the front page of medical records in the process of hospital performance reform in a certain hospital. Firstly, the internal departments are divided into physician groups based on " attending physicians" and the hospital performance evaluation is subdivided into physician groups. Secondly, through the extraction and analysis of the data on the first page of medical records, the assessment indicators are selected from the workload, work quality, service efficiency,medical expenses and other dimensions to comprehensively reflect the operation of in-patient medical care. Third,monthly assessment results are calculated by using hospitalization medical record data, which is used to evaluate the performance of the department in the current month and cash in the monthly performance, so as to realize the effective connection between performance assessment and allocation. Through continuous optimization of the hospital performance evaluation system, departments are urged to further strengthen the diagnosis and treatment of difficult and miscellaneous diseases, improve the service ability of hospitalization, standardize the filling of the first page of medical records, and gradually realize the refinement of hospital management in a certain hospital.
