对比探究T2 Mapping与常规序列对早期软骨损伤的诊断价值

摘要
目的:探讨对比探究T2 Mapping与常规序列对早期软骨损伤的诊断价值。方法:选取2017年10月-2018年10月本院收治的因膝关节炎或膝关节外伤需接受膝关节镜手术治疗的50例患者,分别采用核磁共振T2 Mapping以及常规序列对其膝关节软组织损伤情况进行诊断,并以膝关节镜手术观察结果作为金标准,分析两种诊断方式的临床应用价值。结果:常规序列、T2 Mapping序列与关节镜检查结果比较差异均无统计学意义(P>0.05);常规序列总体检查、Ⅰ级、Ⅱ级、Ⅲ级以及Ⅳ级软骨损伤检查灵敏度均明显低于T2 Mapping序列(P<0.05),0级软骨损伤检查灵敏度高于T2 Mapping序列(P<0.05);常规序列Ⅰ级、Ⅱ级、Ⅲ级以及Ⅳ级软骨损伤检查特异度方面与T2 Mapping序列检查比较差异均无统计学意义(P>0.05),0级软骨损伤特异度方面高于T2 Mapping序列(P<0.05)。结论:在针对膝关节软骨损伤诊断方面,T2 Mapping序列的灵敏度更高,因此在早期软骨损伤的诊断中可以广泛应用T2 Mapping序列。
        Objective:To explore the diagnostic value of T2 Mapping and routine sequence for early cartilage injury.Method:A total of 50 patients with cartilage injury who should be treated with the arthroscopic surgery from October 2017 to October 2018 in our hospital were selected,MR T2 Mapping and routine sequence were used to diagnose soft tissue injury of knee joint,the results of arthroscopic knee surgery were used as the gold standard,the clinical application value of two diagnostic methods were analyzed.Result:There were no significant differences in routine sequence,T2 Mapping sequence and arthroscopy results(P>0.05).The sensitivity in general inspection,Ⅰ,Ⅱ,Ⅲ and Ⅳ cartilage damage of routine sequence were significantly lower than those of T2 Mapping sequence(P<0.05),the sensitivity in grade 0 cartilage damage inspection was higher than that of T2 Mapping sequence(P<0.05).The specific in Ⅰ,Ⅱ,Ⅲ and Ⅳ cartilage damage of routine sequence compared with T2 Mapping sequence had no obvious differences(P>0.05),the specific in grade 0 cartilage damage was higher than that of T2 Mapping sequence(P<0.05).Conclusion:T2 Mapping sequence is more sensitive to the diagnosis of cartilage injury in knee joint,so it can be widely used in the diagnosis of early cartilage injury.
