成年男性高血压前期患者左心房功能观察

摘要
目的观察成年男性高血压前期患者左心房的功能。方法 62例高血压前期患者分为高血压前期一阶段组(病例1组,收缩压120~129 mm Hg和且舒张压<80 mm Hg)、高血压前期二阶段组(病例2组,收缩压130~139 mm Hg和/或舒张压80~89 mm Hg)各31例,同时将35例理想血压者(收缩压<120 mm Hg和舒张压<80 mmHg)作为对照组。采用GE Vivid E9彩色超声心动图诊断测量并记录左心二维参数左心房前后径(LAD)、左心室舒张末期前后径(LVEDd)、室间隔厚度(IVST)、左室后壁厚度(LVPET)、二尖瓣瓣口舒张期前向血流峰值速度E、A值、二尖瓣瓣环间隔侧舒张期峰值运动速度E',采用RT-3DE检测并测算各组左心三维参数左心房最大容积(LAVmax)、左心房收缩前容积(LAVpre)、左心房最小容积(LAVmin)、左房总射血分数(LAEF%)、左房主动射血分数(LAa%)及左房被动射血分数(LAp%)。结果与对照组比较,病例1组LAa%增大(P <0. 05);病例2组IVST、LVPWT增厚LAVmax、LAVpre、LASV、LAEF%、LAa%均增大,LAp%均减小(P均<0. 05)。与病例1组比较,病例2组IVST增厚,LAVmax、LAVpre、LASV、LAEF%、LAa%均增大,LAp%均减小(P均<0. 05)。结论成年男性高血压前期患者LAa%、LAVmax、LAVpre、LASV、LAEF%、LAa%均增大,IVST、LVPWT增厚,LAp%均减小。
