摘要
目的探讨Miltenberger系列混合抗体的分离鉴定方法及其在输血中的临床意义。方法首先用Mia抗原阳性的抗体筛选细胞对患者血清进行抗体筛查,并使用谱细胞及Miltenberger系列的GP.Vw、GP.Hut、GP.Mur、GP.Hil、GP.Bun细胞对患者血清进行初步鉴定;随后利用Miltenberger系列的GP.Mur,GP.Hut和GP.Vw细胞分别对患者血清进行吸收试验,并分离吸收后的上清液;最后用Miltenberger系列的GP.Vw、GP.Hut、GP.Mur、GP.Bun细胞分别对吸收后的上清液进行抗体特异性鉴定。结果患者血型为A型,DCCee,Mia(-),Mur(-);患者血清分别与GP.Mur,GP.Hut和GP.Vw细胞吸收分离后,鉴定结果为抗-Mia、抗-Mur、抗-Vw和抗-Hut的混合抗体,同时可能合并存在抗-MUT。结论抗体筛选细胞中包含Mia抗原阳性的红细胞可以大大提高Miltenberger系列抗体的检出率。利用不同表型细胞对待检血清进行吸收,可以实现对Miltenberger系列混合抗体的分离和鉴定,为患者选择相应抗原阴性的血液配型及输血,确保临床输血安全。
        Objective To analyze the identification method of Miltenberger series mixed antibodies and their clinical significance in blood transfusion.Methods Serum antibody was screened and identified initially with Mia positive screening cells,panel cells and Miltenberger series GP.Vw,GP.Hut,GP.Mur,GP.Hil,GP.Bun RBCs.Related Miltenberger series RBCs(GP.Mur,GP.Hut and GP.Vw) were used in absorption test to separate mixed antibodies.The specificity of antibody in the absorbed sera can be determined by reacting with Miltenberger series panel cells separately.Results The patient' s blood group was determined to be A,CCDee,Mia(-),Mur(-).The mixed antibodies were identified as anti-Mia,antiMur,anti-Vw,anti-Hut and probably anti-MUT using the RBCs with GP.Mur,GP.Hut and GP.Vw separately in the absorption test.Conclusion Using screening cells with Miaantigen positive can improve the finding of Miltenberger series antibody.Related cells used in absorption test to separate antibodies can help to identify mixed antibodies.To ensure the safety of blood transfusion,the patients need to be transfused with blood containing corresponding and negative antigens.
