氨氯地平阿托伐他汀钙片治疗高血压伴发冠心病效果及对颈动脉粥样硬化和血清Hcy水平的影响

摘要
目的观察氨氯地平阿托伐他汀钙片在高血压伴发冠心病患者中的应用价值。方法采用随机数字表法将2015年8月至2016年8月我科收治的124例高血压合并冠心病患者分为观察组和对照组,各62例。两组均予以冠心病高血压常规治疗,其中观察组予以氨氯地平阿托伐他汀钙片治疗,对照组予以氨氯地平片治疗。比较治疗3个月后两组患者治疗效果(血压、血脂、心肌缺血改善率)差异及治疗前与治疗3个月后两组患者颈动脉内膜中层厚度(IMT)和血清Hcy水平差异。结果治疗3个月后,观察组各项血压、血脂水平均显著低于对照组(P<0.05),心肌缺血改善率明显高于对照组(P<0.05)。治疗3个月后,两组患者颈动脉IMT、血清Hcy水平均较治疗前降低,且观察组显著低于对照组(P<0.05)。结论氨氯地平阿托伐他汀钙片治疗高血压伴发冠心病患者疗效明显,能显著降低患者血压、血脂,且能降低血清Hcy水平、减轻血管炎症反应、防止动脉斑块形成,延缓颈动脉粥样硬化进展、改善患者心肌缺血状态。
Objective To investigate the application value of Amlodipine atorvastatin calcium tablets in the treatment of patients with hypertension and coronary heart disease.Methods A total of 124 patients with hypertension complicated with coronary heart disease treated in our department from August 2015 to August 2016 were randomly divided into observation group and control group,with 62 cases in each group.Both groups were treated with conventional therapy.The observation group was treated with Amlodipine atorvastatin calcium tablets,and the control group was treated with amlodipine tablets.At 3 months after treatment,the therapeutic effect(blood pressure,blood lipids,myocardial ischemia improvement rate)of the two groups were compared.The differences of IMT and serum Hcy level before treatment and at 3 months after treatment between the two groups were compared.Results After3 months of treatment,the blood pressure and blood lipid levels in the observation group were significantly lower than those in the control group(P<0.05),and the myocardial ischemia improvement rate was significantly higher than that in the control group(P<0.05).After 3 months of treatment,carotid IMT and serum Hcy levels in the two groups were lower than those before treatment,and the observation group was significantly lower than that of the control group(P<0.05).Conclusion Amlodipine atorvastatin calcium tablet is effective in the treatment of hypertension with coronary heart disease,it can significantly reduce blood pressure,blood lipids,and serum Hcy levels,it can reduce vascular inflammation,prevent the formation of arterial plaque,delay the carotid artery progression of atherosclerosis and improve the myocardial ischemia of patients.
