血管生成素样蛋白betatrophin水平与高血压患者血压变异性和血压昼夜节律的关系

摘要
目的观察血清血管生成素样蛋白betatrophin水平与高血压患者血压变异性(BPV)和血压昼夜节律的关系。方法入选2016年10月—2018年10月原发性高血压患者162例为观察组,同期年龄和性别相匹配的体检健康者150人为对照组;检测两组人群血清betatrophin水平,受试者均行24 h动态血压监测,比较两组间BPV和昼夜节律的差异。结果观察组血清betatrophin水平高于对照组[(563.24±51.26)比(285.37±27.18)ng/L,P<0.05];观察组24 h平均收缩压(24hSBP)、日间平均收缩压(dSBP)、夜间平均收缩压(nSBP)、6:00-9:00平均收缩压(mSBP)高于对照组,而24 h平均舒张压(24hDBP)、日间平均舒张压(dDBP)、夜间平均舒张压(nDBP)、6:00-9:00平均舒张压(mDBP)低于对照组(均P<0.05);观察组24 h收缩压标准差(24hSBPSD)、24 h舒张压标准差(24hDBPSD)、夜间舒张压标准差(nDBPSD)、6:00-9:00收缩压标准差(mSBPSD)与对照组相比差异有统计学意义(均P<0.05);观察组昼夜节律异常率高于对照组(34.0%比15.3%,χ~2=14.40,P<0.01)。Pearson相关分析显示,高血压患者血清betatrophin水平与24hSBPSD、mSBPSD呈正相关(r=0.563,0.572;均P<0.05),而与24hDBPSD、nDBPSD呈负相关(r=-0.528,-0.503;均P<0.05)。多元线性回归分析显示,高血压患者血清betatrophin水平是24hSBPSD、mSBPSD、24hDBPSD、nDBPSD和夜间血压下降率的影响因素(均P<0.05)。结论血清betatrophin水平与高血压患者BPV和血压昼夜节律异常相关。
