氧气驱动雾化吸入联合无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭的临床观察

摘要
目的探讨氧气驱动雾化吸入联合无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的临床疗效。方法选取2016年5月至2017年11月期间本院收治的78例COPD合并Ⅱ型呼吸衰竭患者,随机分为对照组和观察组,各39例,对照组给予常规临床治疗,而观察组在对照组的基础上采用氧气驱动雾化吸入联合无创呼吸机治疗。观察并比较两组生命体征和动脉血气指标、肺功能以及临床治疗总有效率。结果治疗后,观察组的心律(HR)、呼吸频率(RR)、动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)和pH等指标均明显优于对照组(P<0.05);观察组1秒用力呼气容积(FEV1)和用力呼气量占用力肺活量比值(FEV1/FVC)改善情况明显较对照组好(P<0.05);对照组和观察组的临床总有效率分别为64.10%和84.62%,观察组明显好于对照组(P<0.05)。结论应用氧气驱动雾化吸入联合无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭患者,可以明显改善心率、呼吸、动脉血气和肺功能,优化疗效,值得在临床推广应用。
        Objective To investigate the clinical efficacy of oxygen-driven nebulization inhalation combined with non-invasive ventilator in the treatment of chronic obstructive pulmonary disease(COPD) with type II respiratory failure. Methods 78 COPD patients with type II respiratory failure admitted to our hospital from May 2016 to November 2017 were randomly divided into control group and observation group, 39 cases in each group. The control group was given routine clinical treatment, while the observation group was treated with oxygen-driven atomization inhalation combined with non-invasive ventilator on the basis of the control group. The vital signs, arterial blood gas index, lung function and total effective rate of clinical treatment were observed and compared between the two groups. Results After treatment, the indexes of heart rate(HR), respiratory rate(RR), arterial partial pressure of oxygen(PaO2), partial pressure of carbon dioxide(PaCO2) and pH in the observation group were significantly better than those in the control group(P<0.05); the improvement of forced expiratory volume(FEV1) and forced expiratory volume occupied forced vital capacity ratio(FEV1/FVC) in the observation group was significantly better than those in the control group(P<0.05). The total clinical effective rates of the observation group were 64.10% and 84.62%, respectively. The observation group was significantly better than the control group(P<0.05). Conclusion Oxygen-driven nebulization inhalation combined with non-invasive ventilator can significantly improve heart rate, respiration, arterial blood gas and pulmonary function in patients with COPD complicated with type II respiratory failure, and optimize the curative effect, which is worthy of clinical application.
