摘要
目的分析短暂性脑缺血性发作(TIA)和脑卒中患者血清基质金属蛋白酶-1(MMP-1)、MMP-3、MMP-9的表达水平。方法本研究选取2016年1月至2017年1月收治的TIA患者100例(TIA组),急性脑卒中患者100例(卒中组),健康体检患者100例(对照组)为研究对象。分析三组研究对象的一般资料,MMP-1、MMP-3、MMP-9基因表达与水平及其相关性。结果 TIA组、卒中组患者在性别、年龄、体质指数(BMI)、糖尿病史及饮酒史与对照组比较,差异无统计学意义(P>0.05),而吸烟史、高血压病史与对照组比较差异具有统计学意义(P<0.05)。TIA组、卒中组患者的血清MMP-1分别为(123.31±8.20)μg/L和(87.12±5.27)μg/L、MMP-3分别为(93.31±8.15)μg/L和(57.64±5.24)μg/L、MMP-9分别为(133.31±8.89)μg/L和(109.56±5.13)μg/L,水平均高于对照组,差异均有统计学意义(P<0.05)。TIA组、卒中组患者的MMP-1基因、MMP-9基因中基因型T/T与等位基因频率T均高于对照组,差异均有统计学意义(P<0.05)。TIA组、卒中组患者的MMP-3基因中基因型6A/6A与等位基因频率6A均高于对照组,差异有统计学意义(P<0.05)。MMP-1、MMP-3、MMP-9水平增高均是TIA组、卒中组患者的独立危险因素,差异有统计学意义(P<0.05)。结论 MMP-1、MMP-3、MMP-9水平检测有助于TIA、脑卒中诊断,且其主要基因位点突变与TIA、脑卒中发生相关,可为临床预防及治疗提供重要指导。
        Objective To analyze the expression of serum matrix metalloproteinase-1(MMP-1),MMP-3 and MMP-9 in transient ischemic attack(TIA)and stroke patients. Methods 100 cases with TIA(TIA group),100 cases with acute stroke(stroke group)and 100 healthy individuals(control group)from January 2016 to January 2017 were selected as study subjects.The expression levels of MMP-1,MMP-3,MMP-9 and the correlation between the expression of MMP-1 and MMP-9 were analyzed. Results There was no significant difference of the sex,age,body mass index(BMI),history of diabetes mellitus and alcohol consumption between the TIA group,stroke group and the control group(P>0.05). The history of smoking,history of hypertension of the TIA group,stroke group and the control group were significantly different(P<0.05). The levels of serum MMP-1 were(123.31±8.20)μg/L and(87.12±5.27)μg/L,MMP-3 were(93.31±8.15)μg/L and(57.64±5.24)μg/L,and MMP-9 were(133.31±8.89)μg/L and(109.56±5.13)μg/L in TIA group and stroke group respectively,which were higher than those in control group(P<0.05). The genotype T/T and allele frequency of MMP-1 gene and MMP-9 in TIA group and stroke group were significantly higher than those in control group(P<0.05). The genotype 6 A/6 A and allele frequency of MMP-3 gene in TIA group and stroke group were higher than those in control group(P<0.05). The levels of MMP-1,MMP-3 and MMP-9 were independent risk factors in TIA group and stroke group(P<0.05). Conclusion The levels of MMP-1,MMP-3 and MMP-9 have early diagnosis value for TIA and stroke,and its main gene mutation can predict the incidence of cerebrovascular disease which could provide guidance for clinical prevention and treatment.
