宫腔镜电切术与开腹手术治疗黏膜下子宫肌瘤的效果比较

摘要
目的比较宫腔镜电切术与开腹手术治疗黏膜下子宫肌瘤的临床效果。方法将我院收治的142例黏膜下子宫肌瘤患者根据手术方法的不同分为宫腔镜组(宫腔镜电切术)与开腹组(开腹手术),各71例。比较两组围手术期指标、术后1个月恢复情况及术后6个月黏膜下肌瘤复发情况。结果宫腔镜组手术时间、术后下床时间、排气时间及住院时间均短于开腹组,术中出血量少于开腹组(P<0.05);两组患者恢复优良率无显著差异(P>0.05);术后随访6个月,宫腔镜组中1例Ⅱ型肌间肌瘤患者术后2个月出现小肌瘤再生,而开腹组无肌瘤组织复发病例。结论宫腔镜电切术与开腹手术治疗黏膜下子宫肌瘤效果相当,但前者可显著缩短患者手术及术后恢复时间,减少术中出血量,值得推广。
Objective To compare the therapeutic effects of hysteroscopic electrotomy and open surgery on patients with submucosal hysteromyoma. Methods A total of 142 patients with submucosal hysteromyoma admitted in our hospital were divided into hysteroscopic group(hysteroscopic electrotomy) and open group(open surgery) according to the different surgical methods, with 71 cases in each group. The perioperative indicators, postoperative recovery at 1 month and the postoperative recurrence of submucosal hysteromyoma at 6 months after operation of the two groups were compared. Results The operation time, postoperative bed time, exhaust time and hospitalization time of the hysteroscopic group were shorter than those in the open group, and the intraoperative bleeding volume was less than that in the open group(P<0.05). The excellent and good rate of recovery in the two groups was no significant difference(P>0.05). After 6 months of follow-up,one case of type Ⅱ submucosal hysteromyoma in the hysteroscopic group occurred myoma regeneration at 2 months after operation, while the open group had no recurrence of myoma tissue. Conclusion Hysteroscopic electrotomy and open surgery has an equivalent effcet in the treatment of submucosal hysteromyoma, but the hysteroscopic electrotomy can significantly shorten the recovery time, reduce the intraoperative bleeding volume and is worthy of promotion.
