《金匮要略》妇人腹痛管窥

摘要
《金匮要略》列妇人三篇,开专论妇科病分类辨治之先河。张仲景从以下几方面论治妊娠腹痛:属阳虚内寒,治以温补元阳,消除阴寒,调和营血,用附子汤;属冲任虚寒,治以温润摄纳,理气和营,补血止血,方用胶艾汤;属肝脾不调,治以调肝理脾,肝脾同调,方用当归芍药散。从以下几方面论治产后腹痛:属血虚寒盛,治以益血散寒,温中止痛,方用当归生姜羊肉汤;属气滞血瘀,治以破血消瘀,用下瘀血汤。从以下几方面论治杂病腹痛:属热结血瘀,治以清热散结,化瘀止痛,用土瓜根散加减;属风血相搏,气滞血凝,治以活血行血,祛风止痛,用红蓝花酒;属血行不畅,兼挟水湿,治以活血行血,燥湿利湿,方用当归芍药散;属营卫不足,虚寒里急,治以温中补虚,和里缓急,方用小建中汤。
There are three articles about women in Essentials from the Golden Cabinet,which is the first to discuss the classification and treatment of gynecological diseases. Zhang Zhong-jing treats abdominal pain of pregnancy from the following aspects: For yang deficiency and internal cold,Aconite Decoctionb is used for nourishing original yang with warming,eliminate yin cold,and harmonizing nutrient-blood. For CV and TV deficiency and cold,Donkey-Hide Gelatin and Mugwort Decoction is used for regulating qi,harmonizing nutrient-blood,reinforcing blood and stopping bleeding with warming and moistening. For liver-spleen disharmony,Chinese Angelica and Peony Powder is used for regulating liver and spleen together. Zhang Zhong-jing treats postpartum abdominal pain from the following aspects: For deficiency of blood and excessive cold,Chinese Angelica,Fresh Ginger and and Goat Meat Decoction is used for tonifying blood and dispersing cold,warming middle energizer and stopping pain. For qi stagnation and blood stasis,Yuxue Decoction is used for removing blood stasis. Zhang Zhong-jing treats miscellaneous abdominal pain from the following aspects: For heat stagnation and blood stasis,Modified Melon Stalk Powder is used for clearing heat and dispersing blood stasis and relieving pain. For syndrome of wind-blood fighting causing qi-blood stagnation,safflower wine is used for promoting blood circulation,dispelling wind and stopping pain. For unsmooth blood circulation,Chinese Angelica and Peony Powder is used for promoting blood circulation and drying dampness. For deficient nutrient and defensive blood,abdominal spasm of deficiency and cold,Minor Center-Fortifying Decoction is used for warming the middle energizer and nourishing deficiency,harmonizing the interior and relieving spasm.
