近10年国际HIV暴露前预防用药研究前沿热点的Citespace文献计量分析

摘要
目的采用Citespace文献计量分析方法对艾滋病病毒(HIV)暴露前预防用药(PrEP)研究热点、分布及前沿演进等方面进行综述和分析,旨在为国内相关研究和防控策略的完善提供参考依据。方法检索2008—2017年Web of science核心数据库中收录的相关文献,利用Citespace软件对检索文献采用关键词系统聚类分析并统计其标题、摘要、关键词和文献记录的标识符等词频构成比、增长率等指标来确定研究前沿的热点。结果共检索符合要求的研究文献788篇,PrEP文献发表量和他引文献频次均呈显著逐年快速增加趋势;被引共现网络显示PrEP研究初期(2008—2013年)以求证PrEP预防效果的临床试验和药物研究为主;高频词汇分析显示,PrEP研究多以传染病流行病学和药理学领域为主;关键词聚13类显示,第一大类研究热点为男男性行为者(MSM)研究人群,2014年之后国际上PrEP研究转向对药物摄入率、血药浓度、服用方式和接受意愿等研究领域。突现词分析表明PrEP研究伊始便是学术热点,并逐年增加诸多研究分支领域。结论近10年国际上PrEP相关研究热点主要集中于其作为新晋预防策略对传播途径和高危人群比例变化后疫情进展的影响,但最新研究热点偏向HIV免疫学、传染病流行病学等基础分析,提示今后我国PrEP相关科学研究需关注和参考高危人群PrEP服药依从性、意识、态度与顾虑等方面特征。
Objective To review and analyze hotspots,distribution and frontier evolution of PrEP research based on Citespace bibliometric analysis to provide reference for relative researches and improve prevention and control strategies in China.Methods We retrieved the literatures from the core database of Web of science from 2008 to 2017 and analyzedPrEP information by Citespace software.Results A total of 788 documents were obtained,and the published PrEP litera-ture and frequency of other cited literatures showed a significant increase year by year.The co-occurrence network showedthat the clinical trials and drug researches of PrEP were the most in the initial researches,and the high-frequency lexical a-nalysis showed that PrEP researches were mainly based on epidemiology and pharmacology of infectious diseases.The key-word cluster analysis showed that after 2014,PrEP researches turned to fields such as drug intake rate,blood concentration,mode of taking,and willingness to accept.The burst detection showed that PrEP was a hot academic topic at the be-ginning of its research,and increased many research branches year by year.Conclusion In recent 10 years,the internation-al research hotspots of PrEP mainly focus on its impacts as a new prevention strategy on the progress of the epidemic afterthe change in the transmission route and proportion of high-risk populations,and the latest researches on basic analysis ofHIV immunology and epidemiology,suggesting thatdomestic PrEP-related scientific researches will be thefeatures of PrEP medication compliance,awareness,atti-tudes,and concerns of high-risk groups with PrEP.
