二甲双胍辅助治疗青春期多囊卵巢综合征(PCOS)的可行性

摘要
目的:探讨二甲双胍辅助治疗青春期PCOS的可行性。方法:选择某院接诊的75例青春期PCOS患者,将其以奇偶数法随机分为D组(n=38)和MD组(n=37),D组以达英-35(Diane-35)治疗,MD组以二甲双胍(Metformin)+达英-35治疗,对比两组临床用药效果、用药前后性激素、FG(Ferriman-Gallway)体毛评分及代谢指标情况。结果:MD组用药总有效率(94.59%)高于D组(73.68%)(P<0.05);用药后两组LH(黄体生成素)、T(睾酮)、PRL(催乳素)和FG评分均低于用药前,E_2(雌二醇)水平高于用药前,且用药后MD组E_2水平高于D组,其他激素指标低于D组(P<0.05);用药后MD组TC、TG、空腹血糖、空腹胰岛素和胰岛素抵抗指数均低于用药前,且低于D组用药后(P<0.05);而D组用药前后各指标均无显著差异(P>0.05)。结论:二甲双胍辅助治疗青春期PCOS能达到较好用药效果,能显著调节体内性激素水平和月经状况,改善糖及脂类代谢。
Objective:To investigate the feasibility of metformin in adjuvant treatment of adolescent PCOS. Methods: 75 adolescent PCOS patients admitted to a hospital were randomly divided into D group(n=38) and MD group(n=37). Group D was treated with Diane-35. MD group was treated with Metformin + Diane-35. The clinical effects, sex hormones, FG hair score and metabolic indicators were compared. Results: The total effective rate of MD group(94.59%) was higher than D group(73.68%)(P<0.05). After treatment, the LH(luteinizing hormone), T(testosterone), PRL(prolactin) and FG scores were lower than before administration, E_2(estradiol) level was higher than before administration, and in group MD after administration, the E_2 level was higher and other hormone indicators were lower than group D(P<0.05); After treatment, the TC, TG, fasting blood glucose, fasting insulin and insulin resistance index of MD group were lower than before treatment, and lower than group D(P<0.05), but there was no significant difference between the indexes before and after treatment in group D(P>0.05). Conclusion: Metformin in the adjuvant treatment of adolescent PCOS can achieve better drug effect, significantly regulate the level of sex hormones and menstrual conditions, improving the metabolism of sugar and lipids.
