血清E_2与E_2、T比值对中老年妇女骨质疏松诊断与病情进展的检测效果分析

摘要
目的:探讨血清雌二醇(estradiol,E_2)与E_2、睾酮(testosterone,T)比值对中老年妇女骨质疏松(osteoporosis,OP)诊断与病情进展的检测效果。方法:回顾性分析自2015年1月至2018年2月接受治疗的中老年女性患者325例,通过双能X线骨密度分析仪检测患者腰椎L_(2-3)、股骨颈两个部位的骨密度(Bone Mineral Density,BMD),骨密度在正常范围内109例作为正常组,骨密度比正常量低1~2.5个标准差(SD)109例为骨量减少的患者作为对照组,骨密度比正常量低2.5以上个标准差(SD)107例为骨质疏松的患者作为观察组。使用酶联免疫吸附法检测患者的血清E_(2、)T值,对E_(2、)T比值进行统计和分析;利用Pearson相关性分析对血清E_2和E_2、T比值与患者骨密度的关系进行分析。结果:观察组患者的骨密度低于对照组和正常组患者(P<0.05);观察组患者的E_2值和E_(2、)T比值低于对照组和骨密度正常组(P<0.05);Pearson相关性分析结果显示E_2值和E_2、T比值与患者骨密度呈明显正相关(P<0.05)。结论:血清E_2与E_2、T比值可以作为临床上诊断中老年妇女骨质疏松症的标准之一,患者E_2与E_2、T水平越低则骨质疏松症就越严重。
        Objective:To investigate the effect of serum estradiol(E2) and E2, testosterone(T) ratio on the diagnosis and progression of osteoporosis(OP) in middle-aged and elderly women. Methods:A retrospective analysis was performed on 325 middle-aged and elderly women treated in our hospital from January 2015 to February 2018. The bones of the lumbar L2-3 and femoral neck were examined by dual-energy X-ray absorptiometry. Bone Mineral Density(BMD), bone mineral density within the normal range of 109 cases as a normal group, bone density lower than normal 1-2.5 standard deviations(SD) 109 patients with bone loss as a control group, bone mineral density. More than 2.5 standard deviations(SD) were lower than normal and 107 patients with osteoporosis were treated as observation groups. The serum E2 and T values were measured by enzyme-linked immunosorbent assay, and the E2 and T ratios were statistically analyzed. The relationship between serum E2, E2 and T ratios and bone mineral density was analyzed using Pearson correlation analysis. Results:The bone density in the observation group was significantly lower than that in the control group and the normal group(P<0.05). The E2 value, E2, T ratio in the observation group were significantly lower than those in the control group and the normal bone density group. The difference was statistically significant(P<0.05). Pearson's correlation analysis showed that the E2 value and E2, T ratio were positively correlated with the patients' bone mineral density, and the difference was statistically significant(P<0.05). Conclusions:The ratio of serum E2 to E2 and T can be used as one of the criteria for the clinical diagnosis of osteoporosis in elderly women. The lower the level of E2 and E2, T, the more severe the osteoporosis.
