摘要
目的探讨血清脂肪因子vaspin、apelin、chemerin与老年高血压血压变异性的相关性。方法选取2014年5月至2015年11月南京市第一医院收治的127例老年高血压患者为研究对象,以同期120名老年健康体检者作为对照,采用酶联免疫吸附法(ELISA)检测所有受试者血清脂肪因子vaspin、apelin、chemerin表达水平;无创性袖带式血压监测仪行24 h动态血压监测,比较两组患者血压变异性;Spearman相关性分析法分析vaspin、apelin和chemerin水平与血压变异性的关系。结果与正常对照相比,老年高血压患者血清低密度脂蛋白胆固醇(LDLC)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和chemerin表达水平均显著升高,脂肪因子vaspin和apelin表达水平显著下降,血压变异性参数均增大,差异均有统计学意义(P<0.05)。老年高血压患者血清中vaspin和apelin表达水平与血液变异性参数24 h收缩压标准差(24 h SSD)、24 h舒张压标准差(24 h DSD)、白天收缩压标准差(dSSD)、白天舒张压标准差(dDSD)均呈明显负相关,chemerin与血液变异性参数24 h SSD、24 h DSD、dSSD、dDSD均呈明显正相关。Vaspin、apelin和chemerin均是影响老年高血压的独立因素。结论老年高血压患者血清脂肪因子vaspin和apelin低表达,chemerin高表达,三者异常表达均与血压变异性相关。
        Objective To investigate the correlations between serum adipokines vaspin,apelin,chemerin with blood pressure variability in elderly patients with hypertension. Methods From May 2014 to November 2015,127 elderly hypertensive patients admitted in our hospital were selected as the study subjects,and 120 elderly healthy people were as the control group. The levels of serum adipokines vaspin, apelin and chemerin were detected by enzyme-linked immunosorbent assay(ELISA). A non-invasive cuff type blood pressure monitor was used to carry out 24 h ambulatory blood pressure monitoring;the blood pressure variability of the two groups was compared. Spearman correlation analysis was used to analyze the relationships between adipokines vaspin,apelin and chemerin levels with blood pressure variability.Results Compared with normal control,the levels of serum LDL-C,IL-6 and TNF-α in elderly hypertensive patients increased significantly;adipokines vaspin and apelin levels were significantly decreased(P<0.05);the expression level of chemerin and the parameters of blood pressure variability increased significantly(P<0.05). The expressions of adipokines vaspin and apelin were negatively correlated with 24 h SSD,24 h DSD,dSSD and dDSD;chemerin was positively correlated with blood variability parameters 24 h SSD,24 h DSD,dSSD and dDSD. Adipokines vaspin,apelin and chemerin were independent factors affecting elderly hypertension. Conclusion The serum adipocytokines vaspin and apelin were expressed in low levels,and chemerin was highly expressed in elderly hypertensive patients;abnormal expression levels of these three factors were associated with blood pressure variability.
