乳腺癌患者血清性激素、IGF-1、VEGF和组织LOX水平与淋巴结转移的相关性分析

摘要
目的分析乳腺癌患者血清性激素、胰岛素样生长因子1(IGF-1)、血管内皮生长因子(VEGF)和组织赖氨酰化酶(LOX)水平与淋巴结转移的相关性。方法回顾性分析2015年2月至2017年12月我院86例行手术治疗的乳腺癌患者临床资料,其中伴淋巴结转移者44例(转移组),无淋巴结转移者42例(无转移组),并将同期入院的40例乳腺良性肿瘤患者纳入良性组。记录3组患者术前血清雌二醇(E2)、睾酮(T)、孕酮(P)、IGF-1、VEGF水平及组织(乳腺癌患者取乳腺癌组织、良性组取良性病变组织)LOX相对表达量差异,并分析乳腺癌患者血清E2、T、P水平、组织LOX相对表达量及IGF-1、VEGF水平与淋巴结转移的相关性。结果 3组术前血清T水平比较,差异无统计学意义(P>0.05);而血清E2、P、IGF-1、VEGF水平及组织LOX相对表达量比较,均为转移组高于无转移组及良性组(P<0.05),且无转移组高于良性组(P<0.05)。术前,无转移组血清IGF-1、VEGF水平低于转移组(P<0.05);术后4周时,2组血清IGF-1、VEGF水平均较术前降低(P<0.05),且无转移组仍低于转移组(P<0.05)。经Spearman相关性分析发现,乳腺癌患者血清T及P水平与其淋巴结转移无相关性(r=-0.049、0.162,P>0.05),而血清E2、IGF-1、VEGF水平、乳腺癌组织LOX相对表达量与其淋巴结转移呈显著正相关(r=0.825、0.865、0.791、0.750,P<0.05)。结论乳腺癌患者术前血清E2、IGF-1、VEGF水平及组织LOX相对表达量与淋巴结转移状态呈正相关,临床可将血清E2、IGF-1、VEGF水平结合乳腺癌组织LOX表达情况作为预测乳腺癌患者预后的指标。
        Objective To analyze the correlation between serum sex hormones levels,insulin-like growth factor 1(IGF-1), vascular endothelial growth factor(VEGF), tissue lysyloxidase(LOX)levels and lymph node metastasis in patients with breast cancer. Methods The clinical data of 86 patients with breast cancer who underwent surgery in our hospital from February 2015 to December 2017 were retrospectively analyzed,including 44 patients with lymph node metastasis(metastasis group)and 42 patients without lymph node metastasis(non-metastasis group). And another 40 patients with benign breast tumors admitted to the hospital in the same period were included in benign group. The levels of serum estradiol(E2),testosterone(T),progesterone(P),IGF-1 and VEGF and relative expression level of tissue(breast cancer tissues from patients with breast cancer,benign lesions tissues from benign group)LOX before operation were recorded in the 3 groups. The correlation between serum E2,T and P levels,relative expression level of tissue LOX,IGF-1 and VEGF levels and lymph node metastasis was analyzed. Results There was no significant difference in serum T level among the 3 groups(P>0.05). The levels of serum E2,P,IGF-1 and VEGF and the relative expression level of tissue LOX in metastasis group were higher than those in non-metastasis group and benign group(P<0.05),and the indexes in non-metastasis group were higher than those in benign group(P<0.05).Before operation,the levels of serum IGF-1 and VEGF in non-metastasis group were lower than those in metastasis group(P<0.05). At 4 weeks after operation,the levels of serum IGF-1 and VEGF in the 2 groups were lower than those before operation(P<0.05),and the levels in non-metastasis group were still lower than those in metastasis group(P<0.05). Spearman correlation analysis found that serum T and P levels were not correlated with lymph node metastasis in patients with breast cancer(r=-0.049,0.162,P>0.05)while serum E2,IGF-1 and VEGF levels and relative expression level of breast cancer tissue LOX were positively correlated with lymph node metastasis(r=0.825,0.865,0.791,0.750,P<0.05). Conclusions The preoperative levels of serum E2,IGF-1 and VEGF and relative expression level of tissue LOX in patients with breast cancer are positively correlated with lymph node metastasis. The serum levels of E2,IGF-1 and VEGF combined with expression level of breast cancer tissue LOX can be used as an index to predict the prognosis of patients with breast cancer.
