强迫症患者的治疗依从性及影响因素分析

摘要
目的探究强迫症患者的治疗依从性以及分析其影响因素。方法对符合ICD-10强迫障碍诊断标准176例来我院门诊的强迫症患者进行调查,根据是否治疗分为接受治疗组和未接受治疗组,采用个人情况及治疗意愿问卷、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)和耶鲁一布朗强迫症状量表(Y-BOCS)对所有患者进行测试,并分析强迫症患者治疗依从性的影响因素。结果由调查结果可以看出,接受治疗的经济状况显著好于未接受治疗组(P<0.05);首次就诊接受治疗及就诊2次、4次、6次及以上者的比例分别为75.6%、70.5%、61.4%、46.0%;接受治疗组与未接受治疗组在病程及职业方面有显著差距(P<0.05);接受治疗组与未接受治疗组在受教育程度、性别、年龄方面无显著差异(P>0.05);未接受治疗组的HAMD、HAMA评分均显著低于接受治疗组(P<0.05),而未接受治疗组的Y-BOCS评分显著低于接受治疗组(P<0.05)。结论强迫症患者的治疗依从性与患者的病程、职业、经济状况及HAMD、HAMA和Y-BOCS的评分有较显著关系。
Objective:To explore the treatment compliance of patients with obsessivecompulsive disorder(OCD) and the analysis of its influencing factors.Methods:of A total of 176 cases of OCD patients conforming to ICD-10 criteria for the diagnosis in outpatient department of our hospital were investigated,according to whether the treatment is divided into treated group and untreated group,the individual situation and the treatment intention questionnaire,Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA) and Yale Brown YBOCS(Y-BOCS) testing for all patients,and analyze the factors influencing the compliance of patients with obsessive-compulsive disorder.Results:From the survey results we can see,treated group the economic situation better than the untreated group(P<0.05);first medical treatment and were 2 times,4 times,6 times and above were 75.6%,70.5%,61.4%,46.0%;treated group and untreated group had significant difference in course of disease and occupation(P<0.05);treated group and untreated group had no significant difference inthe level of education,age and sex(P > 0.05);the untreated group,HAMA scores were significantly lower in HAMD treated group(P<0.05),but did not accept the Y-BOCS score of treatment group was significandy lower than treatment group(P<0.05).Conclusion:OCD patients' treatment compliance has asignificant relationship with course of disease,occupation,economic status and HAMD,HAMA and Y-BOCS score.
