摘要
目的:急性颅脑外伤后,机体由于相对缺氧,引起细胞无氧酵解增加,乳酸生成增多,动脉血乳酸及乳酸清除率在一定程度上反应了病情严重程度,对患者预后有一定的关系。本文旨在探讨动脉血乳酸及乳酸清除率在评估颅脑外伤患者预后的价值。方法:回顾性分析我院急诊抢救室2015年2月至2015年11月收治的颅脑外伤患者94例,按格拉斯哥昏迷评分(GCS)分值分为轻度组(13-15分)、中度组(9-12分)和重度组(3-8分),并根据预后分为死亡组和存活组,在患者入院时立即对患者进行GCS评分、动脉血乳酸测定,在入院6小时后再次测定动脉血乳酸浓度,计算其动脉血乳酸清除率。分别比较各组的动脉血乳酸值及乳酸清除率。结果:轻、中、重三组动脉血乳酸值均不同(F=19.99,p<0.01),且6h乳酸清除率也不同(F=6.21,P<0.01)。死亡组的动脉血乳酸值(4.20±1.36)明显高于存活组(1.58±0.93),t=-9.78,P<0.01。死亡组的6h乳酸清除率(31.'73%±12.84%)明显低于存活组(46.25%±12.01%),t=4.55,p<0.01。重度组的死亡率明显高于其余两组,c~2=53.92,P<0.01。结论:在颅脑外伤的患者中,动脉血乳酸及6h乳酸清除率能较好的评估患者的病情程度及预后,在临床中有重要应用价值。
Objective:After acute brain injure,anerobic glycolysis increase due to relative hypoxia,then lactate was produced more,To a certain extent,arterial blood lactate and lactate clearance rate reflect the illness severity.There are some certain relations in the prognosis of patients.We aimed to investigate the prognosis value of arterial blood lactate and lactate clearance rate in patients with brain injure.Methods:This study included 94 cases of brain injure treated in the Department of Emergency from February 2015 to November 2015.Based on the Glasgow Coma Scale,we divided the patients into lighter group(13-15),medium group(9-12) and severe group(3-8).We also divided the patients into death group and survival group according to the end of prognosis.Once patients admitted to hospital,GCS,Arterial blood lactate and 6h lactate were obtained.Then calculate lactate clearance rate,and to compare arterial blood lactate and lactate clearance rate of each groups.Results:There were significant differences in arterial blood lactate(F=19.99,P<0.01) and lactate clearance rate(F=6.21,PO.01)between lighter group,medium group and severe group.The arterial blood lactate of death group(4.20± 1.36) was significant higher than survival group(1.58±0.93),t=-9.78,P<0.01.The lactate clearance rate of death group(31.73%± 12.84%) was significant lower than survival group(46.25%± 12.01%),t=4.55,P<0.01.The mortality of severe group was significant higher than other two groups,c~2=53.92,P<0.01.Conclusion:Arterial blood lactate and lactate clearance rate can evaluate the severity of illness and prognosis with the patients of brain injure,there is important application value in clinical work.
