微创方法与常规开颅血肿清除术在高血压脑出血中的应用比较

摘要
目的:比较微创方法与常规开颅血肿清除术在高血压脑出血中的应用效果。方法:选取2014年3月-2015年10月在我院诊治的高血压脑出血患者76例,将患者依据随机方式分为神经内镜微创手术组(微创组)和开颅血肿清除术组(开颅组),各组为38例,就两组患者手术治疗效果予以比较分析。结果:微创组术中平均出血量及手术所需时间相比于开颅组,均少于后者且差异明显(P<0.05);微创组血肿清除率相比于开颅组,高于后者且差异显著(P<0.05)。微创组患者中V级及Ⅳ级相比于开颅组,明显高于后者且差异存在统计学意义(P<0.05),其中V级组间比较差异明显(P<0.05)。结论:针对高血压脑出血患者,对其采用微创方法即神经内镜微创手术予以治疗,相比于开颅血肿清除术,效果更为确切,可缩短病程,可保障患者预后改善,在临床中具有很好应用及推广价值。
Objective:To compare the effect of minimally invasive surgery and routine craniotomy in hypertensive intracerebral hemorrhage.Methods:76 case of hypertensive cerebral hemorrhage patients were selected from March 2014 to Oct 2015 for this study and randomly divided into two groups:minimally invasive endoscopic surgery group(minimally invasive group) and craniotomy hematoma removal group(craniotomy group),each group have 38 cases,to compare and analysis treatment effect of the two groups.Results:The average amount of bleeding and operation time in minimally invasive group were less than that in the craniotomy group(P<0.05).The hematoma clearance rate of minimally invasive group was higher than that in the craniotomy group(P<0.05).Patients in the minimally invasive group in class Ⅴ and Ⅳ compared to craniotomy group was significantly higher than that of the latter and the difference had statistical significance(P<0.05),which between the Ⅴ-grade group significant differences(P<0.05).Conclusion:Compared to craniotomy hematoma removal,for patient with hypertensive cerebral hemorrhage,the effect of using of minimally invasive treatment is better,which short the course of disease and improve patient prognosis,which worth of clinical application.
