环境剂量下低水平砷暴露与正常精子不育的风险相关性研究

人类在普通环境中也通过各种途径(饮水,食物,吸入性颗粒物等)暴露于砷1,2,与砷相关的生殖毒性已经进行了很多研究,但关于砷对正常精子不育风险的影响还鲜有报道。本次研究目的是探究环境剂量下低水平砷暴露与正常精子不育的风险相的关性。病例组和对照组各有101位精子正常不育患者及61正常生育男性组成。使用HPLC-ICP-MS联用技术测定尿液中五种砷形态浓度,包括五价无机砷(Asi V),三价无机砷(AsiⅢ),一甲基化砷(MMA),二甲基化砷(DMA),砷甜菜碱(As B)。同时测定了精子的参数,包括精液体积,精子浓度,精子活力,精子前向运动能力。统计发现,在病例组中Asi V、As B、MMA、DMA、总无机砷及总砷的浓度都显著高于对照组。病例组尿液中Asi V的浓度是对照组尿液中的二十倍。此外,与对照组相比,病例组中存在较高的砷氧化还原指数(Asi V/AsiⅢ)和较低的砷一甲基化指数(PMI=MMA/Asi);与尿液中Asi V浓度较低和砷一甲基化指数较高的受试者相比,尿液中有更高的Asi V浓度和较低砷一甲基化指数的受试者具有更高的正常精子不育风险。本研究结果表明,环境剂量下低水平砷暴露与正常精子不育的风险呈现正相关性,且人体内砷代谢过程的紊乱能对男性生殖能力产生损伤。这些发现为环境剂量下低水平砷暴露与正常精子不育的风险存在相关性提供证据。
In this study,we aimed to investigate the relationship between low-level environmental arsenic exposure and normozoospermic infertility risk.Five urinary arsenic species were quantitatively measured.The observed concentrations of AsiⅤ,As B,MMA,DMA,total inorganic arsenic and total arsenic were significantly higher in case group than those of control group.Moreover,higher indice of redox and lower primary arsenic methylation index were observed for case group.Furthermore,the urine AsiⅤ level and PMI were most significantly associated with normozoospermic infertility risk among the observations.Specifically,in comparison with the first quartile,thesubjects with higher AsiⅤ levels were more likely to exhibit normozoospermic infertility with increasing adjusted odds ratios.Similarly,a concentration-dependent increase of AOR was also observed for PMI.These findings provide evidences that low-level environmental arsenic exposure was positively associated with normozoospermic male infertility risk.
