凝血、纤溶实验室指标在监测妊娠期高血压疾病患者中的临床意义

摘要
目的:检测健康足月妊娠、妊娠期高血压、子痫前期孕妇的凝血、纤溶实验室指标,探讨其在妊娠期高血压疾病患者的诊断、治疗和预防中的临床意义。方法:选取健康足月妊娠孕妇48例、患妊娠期高血压孕妇44例、患轻度子痫前期孕妇46例、患重度子痫前期孕妇42例。进行血浆凝血酶原时间(PT)、部分活化凝血酶原时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)等凝血指标和D二聚体、纤维蛋白降解产物(FDP)等纤溶指标的检测。结果:与健康足月妊娠孕妇组比较,患子痫前期孕妇组PT、APTT、TT明显缩短,FIB、D二聚体、纤溶二聚体、FDP明显升高,差异有统计学意义(P<0.05),随疾病进展差异越显著。患妊娠期高血压孕妇组与健康足月妊娠孕妇组凝血、纤溶指标比较,差异无统计学意义(P>0.05)。结论:健康足月妊娠孕妇的血液处于高凝状态,患子痫前期孕妇的血液处于异常高凝状态,对妊娠期高血压疾病患者进行凝血、纤溶指标检测,对患者病情评估、监测和预防有重要临床意义。
Objective:testing the coagulation fibrinolysis laboratory index of full-term pregnancy,HDCP and preeclampsia pregnancies,and discussing the clinical significance of the diagnosis,treatment and prevention of HDCP patients.Methods:select 48 samples of full-term pregnancies,44 samples of HDCP,46 samples of mild preeclampsia pregnancies and 42 samples of severe preeclampsia pregnancies,to test their coagulation index(i.e.PT,APPT,TT,FIB,etc.)and fibrinolysis index(i.e.D-Dimer,FIP,etc.).Result:compared to the full-term pregnancy group,the PT,APTT,TT index of mild preeclampsia pregnancy group were significandy shorter,and the FIB,D-Dimer,fibrinolytic dimer and FDP index of such group increased remarkably.The difference was statistically significant(P < 0.05) and such disparity was more distinct with the progress of disease.With regard to the comparisons of the coagulation fibrinolysis index between the full-term pregnancy group and HDCP group,the difference has no statistically significance(P > 0.05).Conclusion:the blood of healthy full-term pregnancy is at a hypercoagulable state,while the blood hypercoagulability of preeclampsia pregnancy is abnormally higher.It has great clinical significance of the diagnosis,detection and prevention of HDCP to test the coagulation fibrinolysis index of HDCP patients.
