超声应用于脐血管前置诊断的临床作用分析

摘要
目的:分析超声在脐血管前置诊断中的临床作用。方法:对我院2012年8月~2016年6月收治的脐血管前置患者进行回顾性研究,将其超声诊断结果和临床手术病理检查结果进行对比,分析超声诊断价值并追踪妊娠结局。结果:脐血管前置病例共18例,产前超声诊断16例,漏诊2例,超声诊断准确率为88.9%。其中单胎妊娠15例(83.3%,15/18),双胎妊娠3例(16.7%,3/18);合并帆状胎盘11例与部分性帆状胎盘1例(66.7%,12/18)、低置胎盘5例(27.8%,5/18)、前置胎盘4例(22.2%,4/18)、副胎盘1例(5.6%,1/18)、分叶胎盘1例(5.6%,1/18)、脐带先露1例(5.6%,1/18)。16例新生儿存活,1例单绒双羊双胎孕30W时两胎儿胎死宫内,1例新生儿重度窒息、缺血缺氧性脑病,家属放弃治疗。结论:超声检查是诊断脐血管前置的可靠方法,具有较高的脐血管前置检出率,及时诊断和处理,能有效降低围产儿死亡率。
Objective: to analyze the clinical effect of ultrasound in the diagnosis of umbilical vascular. Methods: patients in our hospital from August 2012 to June 2016 were preumbilical vessels were studied retrospectively, the ultrasound diagnosis and pathological examination outcomings were compared, analysis of the diagnostic value of ultrasonography and trackthe outcome of pregnancy. Results: a total of 18 cases, 16 cases of prenatal ultrasound diagnosis, 2 cases of missed diagnosis, the accuracy rate of ultrasound diagnosis was 88.9%. The singlepregnancy in 15 cases(83.3%, 15 / 18), 3 cases of twin pregnancy(16.7%, 3 / 18); 1 cases with 11 cases of velamentous placenta and part of velamentous placenta(66.7%, 12 / 18), 5 cases ofplacenta lower(27.8%, 5 / 18), placenta previa 4 cases(22.2%, 4 / 18), 1 cases of placenta(5.6%, 1 / 18), 1 cases of placenta(5.6%, 1 / 18), 1 cases of presentation of umbilical cord(5.6%, 1 / 18). 16 cases of neonatal survival, 1 cases of twin pregnancy 30 W cashmere Shuang Yang two fetal intrauterine fetal death, 1 cases of severe neonatal asphyxia, hypoxic-ischemic encephalopathy, familymembers to give up treatment. Conclusion: ultrasonography is a reliable approach for the diagnosis of umbilical vessels. It has a high rate of umbilical blood vessel detection, timely diagnosis andtreatment, which can effectively reduce the perinatal mortality rate.
