摘要
目的回顾性分析三维腔内直肠超声在直肠癌局部T分期及淋巴结N分期中诊断的准确性及可靠性。方法对60例直肠癌患者进行3-DERUS检查,并与手术病理结果对照,对该检查方法诊断准确性进行统计分析。结果 1.大部分直肠壁肿瘤呈侵润型表现,部分为肿块型;三维超声总体诊断准确率为95%(57/60),过度分期2例,分期不足1例。uT1、uT2、uT、uT4诊断准确性分别为98.3%、98.3%、96.7%及96.7%。诊断敏感性分别为85.7%、100%、96.9%及88.9%。诊断特异性分别为100%、97.9%、96.4%及98.0%。2.直肠周围可见椭圆形低回声结节,大小约0.3-0.7cm,回声较均匀,形态较规则,边界光整;三维超声N分期诊断准确率为100%。结论 3-DERUS在直肠癌局部T分期及淋巴结N分期中具有独特的优越性。
Objective To retrospectively analyze the accuracy and reliability of three-dimensional ultrasonography in the diagnosis of local T staging and N staging of colorectal cancer. Methods 3-DERUS was performed in 60 patients with rectal cancer, and the results were compared with the pathologic findings. The diagnostic accuracy of the method was statistically analyzed. Results 1.most of the rectal cancer showed invasion, part of was mass type, the accuracy rate of three-dimensional ultrasound diagnosis was 95%(57/60), 2 cases of overstage, and 1 cases were understage. The diagnostic accuracy of u T1, u T2, u T and u T4 were 98.3%, 98.3%, 96.7% and 96.7%, respectively. Diagnostic sensitivity was 85.7%, 100%, 96.9%, and 88.9%, respectively. Diagnostic specificity was 100%, 97.9%, 96.4%, and 98%, respectively. 2. the rectum around showed oval shaped low echo nodules with the size of about 0.3-0.7cm, the echo is more uniform; three-dimensional ultrasound N staging diagnosis accuracy rate is 100%. Conclusions CEUS has high value in diagnosing hepatic lesions.
