丹参注射液对兔KOA早中期软骨细胞增殖和凋亡的干预研究

摘要
目的:经Hulth造模法造模建立兔膝骨关节炎早中期模型的同时,观察应用丹参注射液经关节腔注射干预后对兔膝骨关节炎关节软骨细胞的凋亡作用,并通过对软骨细胞中IGF-1,TGF-β1和Ⅱ型胶原的表达变化进行测定,进一步探讨和阐述丹参注射液治疗早中期膝骨关节炎的可能作用机理。方法:将40只成年雄性新西兰兔,完全数字随机分为正常组,模型组,丹参注射液治疗组和玻璃脂酸钠注射液治疗组,每组10只。除正常组外其余各组按Hulth造模方式建立膝骨关节炎模型,治疗组分别应用丹参注射液和玻璃酸钠注射液进行5次关节腔注射治疗,并于膝骨关节炎早中期处死实验动物并取材,进行组织病理学观察,软骨细胞凋亡检测,以及Ⅱ型胶原,IGF-1及TGF-β1的免疫组化检测。结果:经组织病理学观察兔膝骨关节炎早中期丹参注射液组mankin’s评分均显著低于早中期模型组(P<0.05);但与玻璃酸钠注射液组比较差异无统计学意义(P>0.05);丹参注射液组与玻璃酸钠注射液组中兔膝骨性关节炎早中期IGF-1,TGF-β1及Ⅱ型胶原均显著高于模型组,差异有统计学意义(P<0.05);凋亡细胞的表达均显著低于模型组,差异有统计学意义(P<0.05);丹参注射液在早中期对凋亡细胞的调控,与玻璃酸钠注射液无明显差异(P>0.05);丹参注射液在中期对TGF-β1的调控,与玻璃酸钠注射液无明显差异(P>0.05)。结论:丹参注射液对兔膝骨性关节炎早中期软骨退变有明显的改善作用,其可能是通过降低凋亡细胞数目,促进Ⅱ型胶原的合成,以及对TGF-β1 ,IGF-1的调控而完成。
Objective: Through hulth method to establish early and middle stages of KOA. Approach injection of Dan shen through intra-articular injection, if it has accommodation for apoptosis and collagen 2, IGF-1,TGF-β1 in different process of KOA. Through animal experiments to verify the efficacy of Danshen injection, to find new ideas and methods for non-surgical treatment of osteoarthritis. Method: 40 adult male New Zealand rabbits were randomly divided into normal group, model group, Dan shen treatment group and sodium hyaluronate treatment group, 10rabbits in each group. Except the normal group the other groups osteoarthritis model established by Hulth model, each treatment group progress Intra-articular injection treatment programs for five times, administration of drug delivery once a week. Progressing histopathological observation, immunohistochemistry IGF-1, TGF-β1 and II collagen Chondrocyte apoptosis detection. Results: Through histopathology Dan shen injection group mankin’s score significance lower than model group (P <0.05);Dan shen injection group and sodium hyaluronate injection group, early and middle stages in KOA IGF-1, TGF-β1 and collagen type II were early and middle stages higher than the model group, the difference has statistical significance (P <0.05), apoptosis cells were significantly lower than model group, the difference has statistical significance (P <0.05). In early and middle stages Dan shen injection group have no- significantly difference from hyaluronate injection group on apoptosis regulation(P >0.05), in middle stages Dan shen injection group have no- significantly difference from hyaluronate injection group on regulation of TGF-β1(P >0.05). Conclusions: Dan shen injection can amelioration cartilage cataplasia in early and middle stages of KOA .it might because of it regulating IGF-1, TGF-β1 and collagen type II, as well as the regulation of apoptotic cells have a clear role in delaying osteoarthritis, and rabbits at middle stage of OA that would significantly reduce the apoptotic cells in cartilage.
