紫茜合剂对CITP患者临床疗效及对FasL的影响

摘要
目的:观察紫茜合剂对慢性免疫性血小板减少症(Chronic immune thrombocytopenia,CITP)用药前后的总症状积分、PLT计数及外周血死亡因子配体Fask的变化,以探讨对CITP的疗效、CITP的病变机理及紫茜合剂对本病的作用环节,为该药进一步应用于临床提供理论依据
     方法:30例CITP患者给予紫茜合剂口服,用药两疗程(每疗程两个月)之后,通过与用药前CITP患者的总症状积分、PLT、FasL做对比分析,进行中西医疗效判定
     结果:l、紫茜合剂对CITP患者的总有效率为86.67%。对疗前疗后患者临床总症状积分和血小板计数结果做统计学分析,结果显示有统计学意义(P<0.01)。2、CITP患者外周血FasL水平较疗前及正常对照组明显升高,经治疗后有所下降,疗前、疗后结果做配对t检验有统计学意义(P<0.01)。
     结论
     1、紫茜合剂对CITP患者有效,总有效率为86.67%。2、经治疗后CITP患者的血小板有所回升;外周血FasL水平有所下降。3、CITP患者FasL高表达可能影响了细胞免疫环节,介导T淋巴细胞的异常凋亡,导致CITP的发生。4、紫茜合剂可能通过降低CITP患者的FasL水平,促使T淋巴细胞正常凋亡,维持机体免疫平衡而达到治疗CITP的目的
Objective:To observe the effects of ZI QIAN compound on the treatment of chronic immune thrombocytopenia (CITP) through the changes of the total symptom score,PLT count and peripheral blood death factor ligand Fasl. To discuss the mechanism of CITP and the effect of ZI QIAN compound to CITP and provide the theoretical basis for the further clinical application of ZI QIAN compound.
     Methods:30 patients with CITP were treated with the ZI QIAN compound for two courses which contain two months, Evaluate the effects of ZI QIAN compound through the comparative analysis of changes of the total symptom score-, PLT count and peripheral blood death factor ligand Fasl.
     Results:
     1.the total effective rate of ZI QIAN compound to the CITP patients is 86.67%. Statistical analysis of the total symptom score and the clinical results of PLT count showed that the results was statistically significant (P<0.01).
     2.Compared with the patients with CITP before the treatment of ZI QIAN compound and control groups.the level of Fasl of the patients after the treatment was decreased significantly (P<0.01).
     Conclusions:
     1.ZI QIAN compound can decrease the total symptom score of the patients with CITP.
     2.The PLT count of the patients with CITP after the treatment was increased but the level of the Fasl was decreased.
     3.The high expression of Fasl of the patients with CITP may affect the cellular immune links and medicate the abnormal apoptosis of T lymphocytes and lead to CITP.
     4.ZI QIAN compound may maintain the normal T lymphocyte apoptosis to play a role in the treatment of CITP through decreasing the level of Fasl of the patients with CITP.
